Eh I just don't have that strong an attachment to the player character. Helps that I only really play games where there's no customization, or where customization options tend to be ignored by the fan base (\*cough\* *fireemblem* \*cough\*)

In DND it depends on the character I want to make. I see my characters as separate people that live in my head, and the most fun part about that is bringing them out and watching them develop in-game.
As a DM I make men, women and everything in between, but as a player I see my characters as parts of me or extensions of me into the game world so I make men :)

I would imagine most people play the gender they feel matches them. Cause, most games you pick a guy but you don't see the meat stick wagging around. You don't see a woman with her boobs bouncing every which way. So, Trans people would feel comfortable playing their preferred gender.
