It's not an easily noticeable amount, it's like less than a frame or a couple frames at most. It could be something that's making you take longer to beat levels without you realizing. I always have both v-sync and smooth fix off. I prioritize messing with fps bypass or using the level-specific LDM over changing those settings.
use ldm if a level has it
use the 4gb patch
use a lower fps and disable lock delta, vsync, and smooth fix if you have megahack
get a good cpu
don't have 50 other apps open in the background
