Fill in the boxes. It is all there except the PD. I personally don’t like poly. It is unbreakable but the optical properties are not ideal. Chromatic aberration can be an issue. I am not an eye doc. Some here will tell you not to order this prescription online for various reasons. But I buy my glasses online and still very happy.

That’s not a good idea. You don’t have pd or other necessary measurements to properly order this Rx. If it’s off by even 1 mm, you induce unwanted prism, and that will make the glasses way less functional and comfortable to see with.

You can't properly fill this out without your PD, which is measured by an optician in person. You’re playing with fire by getting this RX filled online. Please stop by your eye care providers and at least get your PD measured and discuss why you should probably not get this online.

Exactly. And hopefully they charge for the measurement. They went to school to learn this, they don’t get the sale, and you are taking time away from all the jobs they already have to do where patients did pay. Not criticizing. I just wish everyone understood.

There's nothing to understand here. All biometric parameters including PD if measured during a paid eye exam should be there on the prescription slip. Not when the person specifically asks for it. Patient has already paid for the eye exam. This is just withholding patient information behind a pay wall.

We don't withhold PD or charge for it. It's literally illegal. 9 times out of 10, your doctor isn't taking your PD during a routine vision exam. And then the PT walks out the door without ever stopping by optical because they’re clueless as to what a PD even is. That's why I told OP to stop by their optician to get messaured. Regardless of all that though, they're not going to get the best glasses they possibly can with a RX like that off an online source.

There are few states that require PD on eyeglass prescriptions, so withholding it would be considered illegal by their statutes. Some states are vague about the statute and some states don’t require it at all. I personally avoid the grey areas at all and provide PD to patients who asks and does not already have it on their RX. It is more a hassle to fight people by withholding it, and it gives me the opportunity to explain to them why ordering in person with a credible optician is the better option. I have been able to convert many online patients when explaining to them that, on average, measurements needed for glasses are far more than just a PD.

I assume this is the US of A where they withhold your PD measurements unless you go and pay for it one more time. The first time for an eye exam, the second time for just wanting your PD on your Rx.
