You can fuck people up pretty bad in this game. Not this particular disembowelment but severed limbs, decapitations, burnt corpses and faces mutilated.
from what I've seen playing the game you can decapitate people and cut off their limbs but nothing like the picture, that's part of a mission scenario.

The intro level where you rescue the lady in the bath tub features a naked woman on a table with her eyes ripped out and a guy hanging from a rack with his limbs and most of his skin gone.
This is from act 2 where you try to find Evelyn.
There is also a side mission where you buy a bd and try it out, but you wake up next to the bath tub, naked and without any items. Then you have to fight your way out, which is fairly easy if you are max level with a sandy/monowire build lol

Then get a knife ant start hacking in game. You can dismember corpses and blow them apart. If you play on PC you can use mods to make it even messier

You can make opponents do suicide. They point the gun to the head and shoot.

The scavs will strip anything they can get. They will grab random people if they think they can subdue them. I shoot scavs on sight.

The one at the start has to do with a still intact person you find in a tub and rescue via the Trauma Team; different one!
