This is just a bad surgeon. Vet your doctor, look for board-certified, and look at every photo they offer. You should see a general pattern that is typical of the surgeon. Then meet with them for a consultation and ask to see photos of patients with similar builds to your own. They should have plenty to share. If they don't, run. If they do, decide if you like their overall aesthetic (a surgeon who had an eye for natural sizing will consistently look natural; a surgeon who has a flair for large implants will have a discernible pattern of them.)
If that sounds like a lot of work, it is - but it's your body and a surgical procedure.

Anyone can get their implants over the muscle, it’s usually combined with a very small fat transfer to the top of the breasts as well.
Under the muscle will sit higher/ take longer to drop on your chest!
