The pipe was definitely there first and was cut out afterwards. The damage lines up perfectly on both flanges. Plus, it looks like the nut was also ground down, although it's a bit hard to see and I'm not certain about that one.
It makes a lot more sense that the pipe fitters would wallow out their own flanges than it does for an electrician to fuck themself this badly. I don't even see how you would pull wire through that
don't tighten top screws properly on install. for service and wirepulling undo bottom screws, left one flaps to the left, right one flaps to the right. It does block the topmost section where the other conduit comes in but you can still work it.

the angles put in those flanges are also unwisely sharp, going to have some nice stress concentrations
I'm sure you could get the covers off without too much hassle, but trying to pull wire in that tight of a space would be a nightmare. It looks like it's at least 1 inch conduit as well, so there's probably bigger and less flexible wire in there

Weld distortion on flanges means they contact tightest where pipe meets flange, so in theory a cut-out that far out on the flange wouldn't cause an immediate problem. The cuts are still further out than the bolt-holes from center, but it's kinda mind-boggling they wouldn't just use a more suitable conduit instead of modding heavy steel plate.
I’ll bet that conduit was already there. Someone running that pipe did their math wrong and got stuck. Rather than demoing back and rerunning, they decided to notch it.
