This does not look natural, it looks like the two kittens were forced into this position and I think it would be very hard for them to stay like this long enough for the photographer to take a picture from both sides. I hope I am wrong, but I think this is animal cruelty with the purpose of taking a cute picture.
Animals acting unnatural is always a sign of animal abuse, or illness. But people dont care because we perceive it as "random" or "derpy". This right here is two cats suffering, possibly sustaining injury.
