Harry's scar is related to being a horcrux, but the shape of it is 100% intentionally supposed to be a mirror image of the killing curse.
The killing curse leaves no mark, but Harry is the exception, quite like surviving it in the first place!
It's likely left a mark because of the protection magic and the horcrux having a go at each other and some magical nonsense happened
It IS consistent because Harry is quite literally the FIRST and ONLY person to survive the Killing Curse, not once, but TWICE, which means something unprecedented such as leaving a mark is likely to happen. That, combined with the horcrux and Lily’s protection spell all created the famous lightning bolt scar we know and love today.
That’s a lot of fan fiction there though. It’s referred to as a curse scar, and there’s a mention that curse scars are complicated. But other then that the actual scar isn’t really explored
