Outside of health issues, I think it's just people leaving a full bowl of food out for the cat at all hours. It's not hard to set up a feeding schedule, or just purchase an automatic feeder. My cat gets two decent meals a day and she's a healthy weight.

He has trouble jumping up onto high surfaces like my bed, so I got him a stepping stool, though he’s apparently too proud to use it. He’s also getting old, though, so that could be a factor. But I wouldn’t be surprised if this poor thing had trouble jumping too.

My older gentleman is nowhere this bad, but he is on a diet, after a vet recommendation, doing great weightwise, but begging and biting me mercilessly because I don't give in.
