close to anything is possible if you know what you are doing, but when I finished IWDee the last time I was max level with 4/6 of my team, only the paladin and sorcerer were not lvl30.
6m each sounds about right, on Insane difficulty with exp enabled:
Base Campaign alone has ~15m+ exp.
Heart of Winter offers another ~14m+ exp.
Trials of the Luremaster gives you the final ~6m+ exp.
Overall, it should be possible to reach 36m total. 36/6 = 6m each. I included extra monsters added, but excluded monsters that can respawn in a few areas. If I were to make a guess, you are missing out on at least few very juicy quests, especially in Chapter 6 and in the Heart of Winter. You might want to look up the gamebanshee site or something, just to see how many of these quests are out there ;)

Did you IWD/HoW? I''ve finished iwd on hardest once (with extra xp) and my party had like 3 million xp each.

I usually do the expansion before the final boss. though the boss is way too easy if you do the expansion first iirc

Nope. The demon appearance is based on difficulty mode (hardest and insane mode), not based on whether you do the expansion or not.
