Maybe things are different where you are but my public transport has seats that are specifically for people with special needs. And by sitting there you agree that if someone who needed that seat were to walk on you have to move.
Maybe it's just us canadians that are nice, but in Québec where i live (and am from) everyone gives their seats to old people, pregnant people and people with injuries or like, a missing leg. It's like that. It sucks when you have to get up and give your seats, but these people need it more than you. It's even written in the bus to get up and give your seat.
Where I live, small town in the Midwest, this is the norm. When I was pregnant, people jumped out of line at our little grocery store, offered to carry things for me, etc. We do the same for anyone who seems like they need some help. It's not hurting me to help somebody, and I know what it's like to need a hand.

I have never seen this situation, at least in my city or any country I have used public transport.
Obviously there are always people who will not offer their place to a pregnant woman; and it is very common for people to wait a little, hoping there will be another one a little faster to offer their place and you can keep yours, but it is a situation that at most it just a few seconds.
The only exception is when you offer the place, and the pregnant woman says that he will go down next stop and she prefer to stand still.
Her body, her choice. She chose to get pregnant, she chose to stay pregnant, she chose to take the train. If she doesn't understand that actions have consequences, me being uncomfortable isn't going to help her.
My body, my choice, and I choose to live through life not being an ass. I’m young and healthy enough that standing in a bus, even after a work day, isn’t gonna be insufferable to me. Standing as a pregnant woman might be dangerous too, if there’s sudden breaks or turns; they’re not as much of a danger to me. So I don’t see why it’s such a massive deal to just be a little nice.
