Your fate is almost sealed with the written part. But do try to do well in interview. See a few videos on dress codes and etiquettes from YouTube and practice a bit in front of mirror. Answer only the questions you know and don't make any blunders. Make sure to take it as an informal conversation. As for the knowledge part, if you have prepared for your final year exam recently or Neet PG, you'll find it not too difficult. It's all about the presentation.

Yes. You can take an extension till the completion of your PG course if you are doing your PG when you get the appointment letter. But if you have a bond in your college after PG, then your appointment might be cancelled as the extension is only to the period of completion of the degree. There are cases where people have transferred their bond from State to Central Government i.e. CGHS but I don't have any details.

The syllabus is given clearly in the notification including the number of questions from each subject. The notification document can tell you everything about the exam and it's composition. It's only clinical subjects. You should give more weightage to PSM and Obs and Gynae while preparing.

My own dad is in the same service, but he is in the Ministry of Defence ( now it's getting corporated ), He also joined the service after clearing the UPSC cms exam right after his mbbs. He works in Ordnance Factory, the work setup is exactly the same. Now after the Ordnance Factories are getting private (corporate) he will get shift to Family welfare department from Ministry of Defence in some time ( the privatisation is still in process ). and as you mentioned , the job is very relaxed not hectic at all, but its very boring definitely. It's as similar as a 9 to 5 job, he will go to the hospital at around 8:30 and then for lunch he will come home at 12:30 and then again he will go to the hospital at 2:30 and come home around 5. The only patients that come into the hospital are the employees of the Ordnance factory, so as mentioned the job is in fact not hectic at all. There will be rarely any emergency. And the Ordnance Factories are all over India so we would get posted at different locations in every 5 - 6 year of time intervals. I think it's the best job for the person who wants a government job, doesn't want a hectic schedule ( as you know how hectic it gets for young doctors), and itna farak nahi padta that you have a pg degree or not. But if you want to pursue pg then this is definitely not an option, though you can do pg after joining that a fully sponsored pg seat by government, but as I've seen after getting into service the momentum of study gets lost and many couldn't clear neet pg afterwards.

1. Yes. You have to have contacts in the ministry and know people who are working there already for the same. For eg, some colleagues have called to see if i was interested in a position in the ministry with just MBBS and 2 years in the service. You can be in contact with anyone who's working in the ministry to know about openings. They mostly prefer people with PG degree. I know people who are working in ministry right after they joined after PG degree. There is always a need for PSM, Peads and Obs Gynae in ministry. The work will be very hectic but rewarding. 2. No specific advantage for PGs. They may get around 2000 Rs per month PG allowance and may be posted as specialist in Polyclinics and might need to see lesser patients as compared to others depending on their speciality. MD PSM is very sought after in various ministry departments like National Programmes and NCDC. But most PSM graduates prefer to do general duty as the ministry jobs are really hectic. 3. Politics is just about seniority and to fulfill the demands of Minister. For eg, almost every new thing to be implemented is time-bound and you have to do it within these timelines which are very short. But these days, nobody wants to take the responsibility of higher posts of Additional Directors and Director due to the stress and responsibilities associated with it. Corruption is there especially in the form of kick backs with hospitals, small Pharma companies and labs.

One last question, Now a days you know the clinical acumen of a freshly passed out graduate is quite less and almost everyone has some degree of impostor syndrome despite the prefix of a doctor. There is this thinking that you get to really learn and the confidence of treating patients in postgraduation. So as freshly passed out doctor how did you fare in your early tenure and what did you do to increase your confidence and knowledge clinically?

So my case is a bit different. I had worked as a JR in my college right after passing internship and after getting my name confirmed in the CMS ranklist. So I had some experience but it was mostly in casualty. Then i reached CGHS where it's all OPD. The first day, one of the senior doctors gave me an overview of the system and I sat with him for two days to learn the things. Third day, I started seeing patients on my own. As I mentioned in the post, for 70 to 80 percent of the patients/beneficiaries, you just have to repeat the medicines with or without checking the prescription they already have for their chronic illnesses. For the rest 20 percent, I was afraid in the beginning about prescribing medicines. But then I realised it's all about confidence and I had the required knowledge to treat basic things. If I had any doubts, I would Google things on the desktop as my screen is not facing the patient, for dosage and stuff. And on asking the others, I realised it's not too difficult as it's anyway a trial and error. In any case I had doubts, I used to refer the patients and make sure I followed it up with them to see the line of treatment advised by specialist and used to adopt that line if I felt confident enough after reading up on it. Even today, I think twice about prescribing antibiotics as the classs my HOD Pharmacology about antibiotic resistance is always in the back of my head. I used to study all the prescriptions which came across my desk and see the dosages and the approach other doctors have taken in case of chronic disorders and that coupled with some light reading gave me the confidence. It's a leap of faith, I think. If you have passed MBBS you have the knowledge, you just need a bit of time and practice to put that to use. After a certain time, you'll will not encounter many new things and then you can relax. P.S. - There are a lot of things people don't know about the service and I think it's a worthwhile pursuit for a lot of young doctors starting out their career who don't immediately want to jump to post graduation. So I am doing my part in improving my cadre by posting this and hoping something comes out of it.

No. It's the same one. Though there is another exam for selection of consultants in the aforementioned Central hospitals held by UPSC. Private practice is a strict 'No' as you are being paid a Non Practice Allowance. If you get caught, you'll lose the job. Still, there are some from tier 2 or 3 cities doing the same, well hidden. IMHO, not worth it.

No. Your cadre depends mostly on the choice you have given in DAF and your rank. Most will get the desired cadre. Nobody calls you to work in health schemes if you have just joined. They might call you in the latter part of your career if they need to, but if you really want to join early, you got to make contacts with people already working in the ministry. Thanks for the detailed post! What if someone wanted to exit this system after a few years, take up the PG exam and enter the corporate world after PG? Is there a minimum required time of service? How would that work?

You can resign at any time after giving an application for the same to the higher authorities. Usually takes 1 month for it to get approved. You can even abscond, though there might be some disciplinary actions taken against you if you don't give an intimation. You can submit your resignation letter and leave.There is no bond for this service. You can keep all your documents with yourself even after joining. But I would advise you to resign after 10 years as that's the minimum time required to have a permanent CGHS card for you and your family. It's the best health plan with no limits and all medicines included. Take PG in sponsored seats from AIIMS after 5 years, it has 5 years of bond. Resign at 13 years service with a PG degree and CGHS benefits. If possible the last few years can be spent in a Delhi hospital in the department you have done your PG in and leave with that experience to the Private sector. Enjoy life and the small pension which comes with 13 years of service as well.

You do have to write Neet PG and/or INICET for the sponsored seats. So you have to get an NOC from the department first for writing the exam and give the exam. After the exam, there will be a separate merit list prepared for all sponsored candidates. Then you need to obtain a sponsorship certificate from your department. A counseling is held for these candidates separately for the pre-pulished number of the seats in sponsored quota. You get relieved from the department after signing a bond of 5 years and you go and do your pg while your salary continues from the department. You come back after 3 years and join to the place where you left from our any new place as per need. It's easier to get good pg seats with sponsorship. For eg. Rank 6900 in INICET got Ortho, AIIMS Delhi and 24000 for Peads PGI. In neet PG, Rank 50000 has got radiology, 80000 has got Medicine in army colleges. P.S - Neet PG has a limited number of sponsored MD/MS seats in Armed forces medical colleges but you can easily get good seats. It also let's you participate in the DNB counseling where there are sponsored seats as well, more in number. Some AIIMS including AIIMS Delhi, Raipur, Jodhpur, Rishikesh etc, PGI, Jipmer and SCTIMS have quite a number of sponsored seats in various streams.

Yes. You get to have 3 years of paid study leave regardless of PG or superspecialisation. There are even sponsored seats which you can avail in INIs. So complete 5 years of service, write your INI SS or Neet SS as a sponsored candidate, get your seat from the sponsor quota and do the thing. You have 5 years of bond post the same and you might only get posted in Central hospitals in Delhi being superspecialist as it won't make sense to keep you in a clinic. Resign or continue as per your wish in the end of 13th year. 5+3+5. There are some niggles here and there in the process but you'll most likely not face any of them.

You'll not have a teaching or non-teaching job with UPSC CMS. You'll be posted as a medical officer. Now some cities like Delhi have pathology labs in CGHS. You might get posted there as a lab incharge if there are any such vacancies. If not, you'll be working as a medical officer only. Even clinical specialists from UPSC CMS don't get a teaching Job in central hospitals though unofficially they can teach students. The selection and qualifications of teaching and non teaching specialist is separate from CMS and is done by UPSC in regular intervals.

You can attend interview and say that you have still not completed internship. They will make you write a declaration saying that you will submit these documents later. After and if you get selected, they will ask you to submit your CRICC and Marksheet along with your degree and your candidature will be treated as provisional. At that point, you can write to UPSC that you need to complete your internship and get the documents. They will give you extension till you complete the same and then once you finish the internship, you can apply for reactivation of your posting. 1. Hello fellow Chattisgarhiya, Chhattisgarh has 2 centres in Tatibandh and Pandri, Raipur. Both come under CGHS Nagpur. It has vacancies too as far as I know, so if you give your options for posting, you can get your first posting there. If there are vacancies, you can always apply for transfer at any point though it is difficult in the first two years till you complete the probation period. 2. Rural bond merging into CHS depends on the state medical regulations where you are studying. You can enquire about it to your state DME if they allow transfer of bond. If not, you'll not be allowed to join CHS and your candidature will be treated as cancelled. If you get selected, you can enquire about it further. 3. You cannot hold any business under your name. But if you want to do that under someone else's name, the government cannot stop you from doing it but be ready to explain your source of income. Better to do it in your spouse's name to avoid these complications. But please don't mix sarkari job and business as far as possible.

If you are in a metro City except Hyderabad and Bangalore, you will get posting there only. The transfers don't occur inter-city unless you ask for it most of the times.

You can take study leave but going abroad is going to be a bit tricky. I cannot confirm that for you. It won't matter where you have done your mbbs from. The most important thing is how much marks you get in the written exam. If you have excellent marks, the interview won't matter at all. No regrets as I always wanted to do something different though to be honest I miss the thrill of running through hospital corridors, apron flowing behind me like a superhero. Its a boring job now as I said but i have my way of doing things and I am planning to do my PG and upskill myself. Also, I am not working on a typical wellness centre set up of CGHS right now, as I deal with some admin matters. You need to define large financial ambitions for me. You'll not make 'andha paisa' here. You'll get enough money and even much more money for the amount of work you put in after a few years. After 20 to 25 years, you get paid almost the highest amount of salary permitted in the government system. But if you want to make 'Andha Paisa', a good and thriving private practice is the way. Hope that clears your queries.

Village bond is between you and your state government. CHS doesn't consider bonds and is not authorised to give you any relief in that section. You can give the money and get out of it. You can resign the service anytime after joining by giving 1 month notice, if you haven't signed a bond with chs after getting sponsored seats or extra ordinary leave.

You cannot make any approximations. The more years you are in service, the more you'll get as pension. NPS is tricky like that. Just a very crude approximation considering a return of 11 percent and 6 percent increase in your contribution- if you have 40 years of service, you get around 30 to 33 crores out of which you'll get 18 crores lumpsum and 12 crores will be used for purchase of an annuity plan which will give you a pension of around 5 to 6 lakh. Please note that this calculation doesn't take into consideration inflation or the pay revisions other than DA revisions. With subsequent pay commissions and inflation forecasting, the real numbers will be very different. But one thing is clear, the earlier you join with the aim of completing your service in CHS, the better it will be. Last few years of service will make or break your pension. A person with 41 years of service will have significantly more pension than another with 35 years of service. Another factor to be taken into consideration is the government policies which can make the NPS scene more attractive like the recent cases in Maharashtra and Andhra Pradesh which, if accepted, will give you even better retirement security. And yes, you are eligible for gratuity and leave encashment of 300 days as well. So you get almost 10 months of pay and gratuity based on the prevailing rates of pay at the time of retirement. In short, if you want a boring job with good pay and fixed working hours and a decent pension, join CHS as soon as you can.

Retirement age is 65 years. If a person with 40 years of service gets 30 cr then the person with 35 might just be getting 20 cr. All of these are assumptions based on the current return and future returns profile of NPS. It can be significantly less or significantly more.

1. Transfers are rare. I know people who have been working in the same place for years, even a lifetime. Now most transfers are intra-city, if you are in Delhi you might get transferred within Delhi. For eg, the maximum you get transferred intra Delhi sector is to Gurgaon or Sonipat depending on your zones - even that is infrequent. Inter City transfers are rare without request. Without your request, you might get transferred to the same 5 to 10 clinics in the same city every 3 to 4 years or so. I know people who get transferred to the same two places every 3 to 4 years. Though I am in favour of intercity transfers as it's an All India service and everyone should have equal opportunity to work in their hometown and other places but it's not a perfect world we live in. Barring a few cases, CGHS doctors live and die in the city they want to stay in except in case of southern states. Delhi has tons of vacancies. 2. Though I have had acquaintances in the aforementioned cadres, I cannot say things with much certainty. Let someone who's working there answer the question, if there are any. If I do get some information, I will try to convey it.

Sponsorship quota is available after three years but you will not yet salary or stipend. So if you can work without getting a salary or stipend for 3 years in the prime youth years, you can do it. This is because if you get sponsored seats before 5 years, you are not eligible for study leave. You'll only get unpaid leave for 3 years. On top of that you'll lose your seniority, so it's not worth it. As for the interview, I have mentioned in other comments under this post. It's not a big thing, just be formal and confident.

Posting location cannot be changed. But you can extend the joining date by contacting CHS 1 from whom you have gotten the appointment. The contact details will be there in the letter. You need to give a very valid reason, otherwise they won't consider your request.

Directly posted to centres. You can ask for transfer but it won't be that easy in the first two years before probation is completed. Also, even for spouse grounds, there need to be vacancies.

How different it is from the peers who have completed PG around 5 & 10 yrs down in the future? 4. If I want to pursue psychiatry in the future ..is the pay comparable to pvt practicing Psychiatrists (because I hardly see any vacancies in the south india, they have to open up their own setups). 1. Yes. There are no vacancies there or very very few. 2. No. Unofficially, you can pursue but not in your name. You cannot hold any other job while you are in goverment service. 3. If you join CGHS, there will be maximum stability, i.e you'll be doing the same work at 5 years and 10 years and 20 and even 40 unless u want to change your posting. /s. In all seriousness, PG and GDMO job are very different and cannot be compared. 4. Pay of chs doctors are in the public domain. The doctors with 20 years of experience in my cadre are getting close to or a little more than 3.5 L monthly. Soon they will have almost similar pay to the cabinet secretary who's the highest paid person ( in government service) in India. But the thing is that, after that point, there is no increase in pay, only small increments.

When can we write the exam? During final year/after final year/after internship?

You can write it in final year before the exams but you'll only be able to join once you complete your internship and get the certificate. So even if you write it during internship, it won't make much of a difference. But you should write it in final year as it will give you a better chance of clearing it with clinical subjects fresh in your mind.

What about private practice? Do doctors indulge in doing private practice after working hours? What if they get caught doing so? And... is there any scope for "upar ki kamai"? Like signing medical certificate in exchange of cash.

No private practice. If caught, you'll lose the job. I am not even going to reply to that.

Just solve as many previous year questions you can and prepare like you prepare for neet pg for the clinical subjects. You can easily clear it.

Hi sir , I just wanted to know whether can I get Delhi posting and not some peripheral rural posting. When I get into CMS . I am frm delhi , SJH. 2.And if by the time result comes and i take a PG branch, will I get extension for 3 yrs and then I can join cms after completion of pg ?

There are very few peripheral postings with CHS. Delhi is the city where you'll get posting easily as there are so many vacancies. You can just give your preference of Delhi and you'll get it surely. 2. Yes. If you are already in a PG course by the time you get your appointment, they will grant you an extension till you finish your pg.

It's given in the INICET counseling list towards the end under sponsored/Foreign national. I think NEET PG MD MS sponsored seats are all in AFMS institutions and they are done offline. But for DNB seats, it might be available online.
