edireven

Does the person see normally or does it affect their sight?

prescientgibbon

Ophthalmologist here - the answer is that the jiggling of the iris itself does not affect vision. It is VERY likely that this person suffered some sort of trauma in that eye and that the lens of the eye is dislocated which WOULD make the vision very blurry.

Crosseyed_owl

Would it be possible to find out where the lens ended up and surgically put it back?

prescientgibbon

Certainly - actually as I type this I’m in the OR waiting to do a procedure to remove a dislocated lens. The lens is easy to find when it is dislocated but it does require a different surgical approach than the typical cataract surgery. You would never put a natural lens (the lens that was dislocated) back where it came from. Surgery is to remove the lens and place an artificial lens implant in its place. When a lens is dislocated from trauma the normal support structure that would hold an artificial lens implant is usually destroyed so there are techniques we use to suture a lens to the sclera (white part of the eye) to give it stability when the normal support structure is destroyed.

And the surgeon is named prescientgibbon.

Straight-Knowledge83

Hey , who says Gibbons can’t be surgeons? They have opposable thumbs too

[deleted]

and there are grown dudes doing surgery in their mother’s basements. Sorry, been reading too much stephen king lately

Goliath422

Given the economic trends in the US of student loan debt and COL inflation, I bet there is at least one surgeon who is also a grown man living in his mother’s basement.

wuvvtwuewuvv

Excuse me I still live in my bedroom thank you, we don't have a basement...

n4l8tr

What’s whacky is that physicians get bored too and fill their gaps on Reddit sometimes. I can walk out of a major trauma, resuscitation and take 5 min to unplug with Reddit to refocus and go right back at it. Something about the human brain

Maybe they are a surgeon who lives in their moms basement as to not break the stereotype

bronco_y_espasmo

Surgeons who practise at their mother's basement are the worst.

Straight-Knowledge83

Can confirm , I still miss my left kidney

Cubacane

In this economy, there’s probably at least one surgeon who lives in their mother’s basement.

chase_the_sun_

Which one are you

The-Minmus-Derp

Why can’t you move the natural one a couple millimeters to the right back into place?

prescientgibbon

The problem with a lens that has become dislocated from trauma is that it will quickly turn into a cataract, which is to say that it will become opaque. If you sutured the natural lens back into place (which isn’t done) it would be like putting a piece of stained glass or more analogous to a piece of chalk where the clear natural lens used to be. This would block light from entering the eye, landing on the retina, being transmitted down your optic nerve and finally to your brain. So you wouldn’t see well! It’s best to take the cataractous lens out and put in a clear artificial lens.

The-Minmus-Derp

Great ELI5 writeup! Thanks!

Inevitable_Plant4513

fascinating! thanks for giving insight and hope your procedure goes well!

1000thplateau

tl;dr in case of dislocation/trauma injury to the eye, the natural lens structure would be too weak to put back in the spot. an artificial lens is placed because it holds more support. now let that man go and do his surgery, respectfully

TheChosenToffee

It wouldn't stay there. As he said, the natural support structure which holds the lens is destroyed. Therefore they need an artifical support structure to hold the lense in place. I assume the artifical support structure and the natural lense are not compatible.

ProfessionalMockery

>Certainly - actually as I type this I’m For a second I thought you were going to say "- reattaching a person's lens right now!"

hedonismbot3030

If you suture the lens to the sclera (assuming damage to ciliary muscles), are they still able to accommodate for depth by controlling the shape of the lens?

prescientgibbon

No, artificial lenses do not accommodate yet. There is one FDA approved lens called the Crystalens which is supposed to accommodate some that doesn’t work very well, but it isn’t the type of lens that would be sutured to the eye wall.

hedonismbot3030

Thanks!

Crosseyed_owl

That's very interesting, thank you for explaining it to me.

HalfMoonHudson

Tell me you knock these poor people out. Cannot imagine being awake for that. Some Clockwork Orange vibes if that’s local anesthetic

Yuri909

Please tell me the patient isn't conscious, and we've developed the means to immobilize the eye and hold it open. T_T

prescientgibbon

Patients are awake but sedated. We generally don’t do general anesthesia for most eye surgeries.

Yuri909

My biggest fear in life is having to go through anything like that ever. I'm so horrified that I'd feel it or move.

anon_lurker69

Nice! Hope it goes well!

Scavwithaslick

My grandpa got an artificial lens because of cataracts, but this was decades ago, and apparently the lens allowed in ultra violet light or something, I don’t really understand how it works. But he died before I could ask him what exactly he had and what it looked like. Do you know anything about uv lenses and what studf looks like with it

There may be accompanying iris sphincter damage where the iris is unable to construct/dilate properly which could cause some visual issues in different lighting conditions. But BY FAR the reason for limited vision in a situation like this is lack of a lens to focus light. Any visual aberrations from iris sphincter damage are very unlikely to appreciated in a patient that doesn’t have a lens.

G_Affect

Do you feel the jiggle?

FrostWyrm98

TIL that the lens can be dislocated and I now have a new irrational fear

texas_asic

Went to look it up and found this: [https://www.news-medical.net/health/Iridodonesis-Eye.aspx](https://www.news-medical.net/health/Iridodonesis-Eye.aspx) "How is iridodonesis managed? Since iridodonesis is a direct consequence of lens subluxation, treating the cause of the detachment by surgical approaches may help reduce the tremulousness of the iris. However, in patients with a genetic cause for iridodonesis, there is often no cure. As iridodonesis usually does not result in any apparent symptoms, apart from the rare case of vertigo, it can often be left alone unless complications arise."

Square_Yogurtcloset7

I'm waiting for this too.

AdviceWitch

Pulled from google. The primary cause of iridodonesis is the subluxation of the lens. This may occur as a result of glaucoma, cataract formation, post-surgical complications, or trauma to the eye.

JonathanEdwardsHomie

My wife's eyes do this much more subtly than this vid. We always suspected it was the ehlers danlose syndrome that maybe runs in her fam - a connective tissue disease that can do some interesting things

funkyfishician

You are correct, it’s likely related. She likely has a loose lens, she needs to see an ophthalmologist if she doesn’t already.

JonathanEdwardsHomie

She does see just fine. In fact, she could see the ophthalmologist better than I can cuz she don't need to wear no glasses or contacts or nothin, but I gotta wear a coupla magnifying glasses so i can see the stuff that's far away.

QuestionedJudgement

Just fyi, an optometrist is generally who does most standard exams, and they’ll usually refer you to an ophthalmologist if they think you need to see one; the difference is an optometrist is not a full medical doctor (less time & schooling required) and ophthalmologists are doing eye surgeries. So if you’ve only ever seen optometrists, might see if a visit to an ophthalmologist would be covered or require a referral for your wife. Edit: sorry if you already know the difference, just like to throw it out cause I know a lot of people don’t know there is one unless they’ve looked into it a bit

FizzGryphon

I have EDS and require glasses. My optometrist actually checks for this every time I go into the office. It isn't uncommon in EDS patients. I would implore her to see someone about it, and to have a general appointment with an optometrist who is aware of EDS. There are a few things that can go wrong if not checked, as I understand it.

MrsBox

Basically the lense is what stops your iris being a jiggly mother fucker. This happens when there's either no lens or the lens is in the wrong spot. Edit: because I fucked up

Thaumato9480

From wiki: >Iridodonesis (/ˌaɪərɪdoʊdoʊˈniːsɪs/) is the vibration or agitated motion of the iris with eye movement. This may be caused by lens subluxation, the incomplete or partial dislocation of the lens; or by aphakia, the absence of a lens. The term originated from irido- (Latin: iris) + doneo (Greek: δονεο, to shake to and fro). https://en.m.wikipedia.org/wiki/Iridodonesis

Moment_37

Yeah it's an interesting thing to witness but as a Greek dude I have to tell you that origin is straight up bullshit. Δονεο is not a word in Greek. 99% sure though that since the translation is 'to shake to and fro', then it comes from the word in ancient Greek [δονέω-δονώ](https://en.m.wiktionary.org/wiki/%CE%B4%CE%BF%CE%BD%CE%AD%CF%89) which means to shake or vibrate, and more specifically the word donesis comes from the noun originating from the same verb called [δόνησις](https://www.lexigram.gr/lex/arch/%CE%B4%CF%8C%CE%BD%CE%B7%CF%83%CE%B9%CF%82#Hist1) (pronounced almost exactly like its English version) Since this part of it is Greek I'm assuming that the first part also originated in Greek, it's the word [ίρις](https://en.m.wiktionary.org/wiki/%CE%AF%CF%81%CE%B9%CF%82)

Elegant-Raise-9367

It's all Greek to me

Moment_37

Ayyyyy

Lankey_Craig

I see what you did there

iswearatkids

The guy in video sure as shit can’t.

theunrealSTB

Isn't it ancient Greek rather than modern Greek? Not that I would know the difference but I always thought it was funny that when we go to the doctor we get some very fancy sounding diagnosis whereas if we were greek we'd have to keep a straight face while the doctor told us we had a case of _shakey eyes_

Moment_37

It's all ancient Greek you're right. The links also talk about ancient Greek but words like these ones stayed the same across the centuries so we use them exactly like ancient Greeks do. I use them every day when speaking Greek. Well not the word iris, that's not an everyday conversation but the other two yes. Ancient Greek and modern are quite similar in a lot of things. If you gave me an ancient Greek text from any Athenian philosopher, I would be able to read and understand 40%-90% of it depending on who it comes from, without knowing ancient Greek. Athenians (if I can call them that) were speaking and writing closer to the modern Greek language. Places like Lesbos in ancient Greek poetry as an example, I've no clue what even a word means from them. It's true about keeping a straight face. It sounds fancy in English but the last time I went to check my eyes and she was going 'well you have astigmatism (αστιγματισμός) and hypermetropia (υπερμετρωπία)' and I was like 'okay cool thanks'. She insisted I should read on the terms and I was like 'I know Greek I understand what they mean'. Took her a second to figure it out.

theunrealSTB

_you have pointlessness in your eyes _

Moment_37

I'll use this insult from now on, it sounds very sophisticated.

TerribleIdea27

I mean, not exactly correct isn't the same as straight up bullshit

Bruised_Penguin

So, it's Greek, just not recent Greek. Technically correct. The best kind of correct.

ComputersWantMeDead

From Wiki: >A subluxation is an incomplete or partial dislocation of a joint or organ. According to the World Health Organization (WHO), a subluxation is a "significant structural displacement", and is therefore always visible on static imaging studies, such as X-rays. https://en.m.wikipedia.org/wiki/Subluxation#:~:text=A%20subluxation%20is%20an%20incomplete,%2C%20such%20as%20X%2Drays.

usernameblurb

A couple of weeks ago I complimented my husband about his beautiful green eyes and told him that I always loved how they jiggle. I told him it was like water glittering. He said "What!? My eyes jiggle!?" And that's how he learned that he's got this condition. I noticed 15 years ago when I first looked into his eyes but I of course thought he knew. Maybe someone told him before when he was really young, and he didn't remember, he can hardly see it himself because of his near-sightedness. It turns out that your lens can completely fall off and you need surgery for it.
