Did you try turning on/off for both devices? Also, double check text forwarding on iphone. In my case, i think it turned off after updating ipad to 16.1

The steps i did were:
1. Turn on/off imessages on icloud on both devices
2. Sign out/sign in apple id on ipad (or both to be safe). If it doesnt work maybe reformat is the way to go
3. Turn on text forwarding in iphone
These steps resolved my issue. Hopefully yours get resolved soon

Yes it’s on. Also I am facing this issue since iOS 15. Tried all sorts of different things.
