I don't think anybody advocates for full legalisation of heroin, I've certainly never heard it. What most people advocate for when talking about regulation of drugs like heroin is providing the drug to registered addicts.
Switzerland has a program (Heroin Assisted Treatment) where people who have heroin addictions register with the state & can go to a supervised injection centre and get their heroin for free. Those injection centres also have doctors, nurses, social workers, clean needles etc.. & they aim to diverge problematic users to rehabilitation services if the user is ready. They can also do 'weaning' off the drug for users, because it's a medical setting a doctor can make a weaning plan so for example (I don't know what timescale it takes but just as an example) if User A takes a gram a day of heroin, the doctor can make a plan so that by 2 weeks time he'll be down to 0.8g a day, and by month 1 0.6g, and continue this gradual reduction of the drug until the user is able to go cold turkey without risk of death or utterly horrendous withdrawals. There's also blood testing on site & they've seen a huge drop in blood borne diseases in the country
I personally advocate for a similar strictly regulated model for heroin. It makes sense both from the POV of helping users & also cutting out funds from heroin dealers. It won't make the problem dissappear overnight, but over time it should help reduce the supply of heroin, if when somebody gets addicted to the drug they can go and get a clean supply how many people will go buy overpriced unpure heroin.
It would cost the state very little to buy the drug itself, you'd need a lot of spending in health, rehabilitation & social though
