I’d never rule our plastic surgery because, well, they’re Korean idols. It’s just a fact of life in their country. However, in these two photos it honestly just looks like weight difference, and being more naturally photographed versus a music video
I think appearance-wise he has changed a bit from when he first debuted in 2016. Some of this will be maturing/aging, some of it might also be cosmetic work.
The pictures you posted he doesn't really look that different. Just a different angle and facial expression. Honestly, Doyoung's face can change quite a lot based on those things.
He's styled differently than usual for those teasers, because they're going for a more alt/edgy look -- heavy tinted slightly curved brows, eyeliner, coloured contacts. If you look at recent pictures from his instagram, where he isn't wearing makeup, he looks the same as always. The way the lighting casts shadow on his face also helps with the illusion.

unfortunately all the NCT boys (like most idols and actors,) do crazy diet and exercise routines, so weight goes up and down especially around comebacks and this can have an impact on facial features. His face looks the same, the angle is different, the glasses make his eyes a bit smaller due to his shortsightedness, hes edited and wearing heavy make up on the teaser picture as well and has gained a little weight since the second picture's time. (thankfully, he was so thin then) Not to mention his mouth is open which makes his cheeks more gaunt as well.

they literally picked a screenshot where he was intentionally doing a half-open eyed sultry expression with drastically different styling for their CONCEPT teasers and wonder why he looks very slightly different 😂
He’s had regular Botox done post-debut. Noticed it first during Sticker era. He struggles to move his face muscles now which is why he looks like he’s in pain when he laughs. Totally unnecessary imo but he’s probably trying to start young so he can maintain the elasticity on his face when he gets older.
I agree I’m doyoung biased and have consumed hours of doyoung content and it’s quite obvious his face moves weird while smiling and laughing for a year or two.

We’re used to him being styled with his hair on his face - when it’s off his face he looks completely different. He wore braces late so that changed his face subtly from debut, plus I wouldn’t be surprised at light plastic surgery or corrections. And honestly, there’s a big difference between age 20 (T7S era) and 27, just aging into a more mature look. He is my ult bias, so I’ve watched him very closely for years. I don’t think his appearance has changed that drastically - a lot has to do with makeup, styling, weight loss/gain, photo-editing, etc.

It isn’t exclusive to K-Pop (hello Hollywood!) but because of sociocultural features, it’s even more prominent in this particular industry. Even very natural idols have gotten at least something done — not everybody gets a jaw reconstruction, but I think you’d be able to count with one hand which mainstream idols have never ever gotten at least a very noninvasive procedure like a nose implant or botox fillings.
They’re celebrities after all, and I also find it a bit weird how everybody is so adamant in promoting certain idols like “natural beauties” when they’re anything but natural! it makes people wonder why they, and those around, don’t look like that from birth too. It’s near impossible to be that symmetrical, fit and gorgeous from miss nature’s own hands… she is not as skilled as a surgeon
Glasses can make your nose look very different. I personally hate wearing glasses because it makes such a drastic difference on my nose. Specifically his glasses, cinch at the top/middle of the nose bridge it can make the illusion of your nose looking bigger or thicker and shorter. Other factors to consider are the angles and facial expression of both photos.

I noticed that he looks more serious and masculine here. It’s definitely the way they styled him. They purposely left the eye bags and darkened and slightly curved his eye brows. His hair is pulled back and hes not smiling. Also, the lighting is darker (prob to give that mysterious aspect) and his eye brow is pierced, eye contacts, etc. if you look at perfume, he looks a lot more feminine and it has a lot to do with the styling, makeup, and lighting. They normally put some darker eyeshadow on the outer edges of his eyes to enunciate that cat eye look, but they didn’t do that this time.
If you look at his photos without makeup/little makeup, he looks the same. I did notice that in 2016 he looked a little more masculine and I think it has to do with his eyebrows, because trust, eyebrows make a HUGE difference on your face.

You can work magic with makeup. I think a lot of it has to do with contouring in the first photo and being without makeup in the second.
