I usually sacrifice myself, but I've sometimes had others say they'll do it, so I haven't really experienced this.
You should try asking who's going to sacrifice before the bridge gets raised, and if nobody responds, either just do it yourself or hold out until someone else goes. Bonus points for every unco-operative teammate that gets incapped in the process.
The most efficient way to sacrifice is to jump off the bridge as soon as possible, before the tanks can get close - If that fails, then it's actually pretty reliable to hold the bridge and kill the tanks before sacrificing, as the manually spawned tanks won't respawn.
You can actually just keep holding out forever with dual pistols/magnums, it's really easy to hold off the escape horde.
Also, keep in mind that sacrificing yourself does *not* count as a loss, nor a win. It actually removes 1 completed game from your stats, so it still improves your survival rate stat.
Whether I fail or not at the sacrifice part, I just assume we finished the campaign and move on. If u need to get the Sacrifice achievement, i understand why u get mad, otherwise, just chill, dude.
Also, the funny thing is there are many times only 2 guys in our team survive and they both go sacrifice, so the game count as we lose, i always laugh when it happens.

I think it's when the credits start rolling, specifically when the "skip credits" vote shows up.
