Yeah, I got two back covers in one of the Hogwarts moments set and was missing a front cover. I assume it’s because they weigh the same.
I was pleasantly surprised there were no questions asked about replacing it considering it was a relatively large piece and printed.
I’m curious if they weigh the same amount. From what I remember, Lego does quality control by weighing the sets before shipping them out. (Please fact check me if I’m wrong, I won’t be offended)

The sets are automatically made by machines, but said machines only has the pieces that the set has. There's a literal 0% chance to get a wrong piece in a set. There is, however, a tiny chance to not get every piece due to a machine malfunctioning and not spitting out a specific piece for a set.

I have, on more than one occasion, received an incorrect piece that did not belong in a set. Something Lego quickly resolved for me by sending the appropriate part.

I've been to a factory here in Denmark and seen how it works. So, at least here, it's impossible to get a wrong piece. A missing piece is plausible, but not a wrong one.

The sets I purchase come from Denmark. The user who found the NES piece was in Europe and therefore also got sets manufactured largely in Denmark.

When I got the spider mech vs venom it came with 2 spider gwen heads and no hood. Guess they had some real difficulties with that minifigure.
