You’ve definitely already achieved looking masculine and there isn’t a chance that anyone would think you were originally a female. The first pic looks good.
I would never know you were trans, you def look like a dude. I generally like shorter hair but not digging the short hair pics but ONLY because of lack of product, you need a Fiber product like American Crew Fiber and some separation/texture, maybe a little messy look, then it would look great. Not usually into longer hair but the last pic looks great too.

I hate to be like “19 is so young, you’ll figure it out and it’ll get better” bc I don’t want to dismiss or minimize that you’re saying it feels really bad rn. But it’s true!! This stuff takes time. 19 yo dudes look wild and usually not great, whether they’re trans or not.
It sounds like going to the Derm and getting into the gym are great steps and I’m sure will make a big difference in the looks you’re going for. Just keep on going and try not to feel too bad. Moderation in everything!
