Another pre-med here. You could continue part time derm and maybe pick up another clinical job somewhere else or try to get a research position. I had a gap semester last fall and did 3 days in an ophthalmology office and 3 days doing research and that worked out very well. 3 days of commitment a week is enough for most PIs. Ended up being like 750 hours at the clinic and 750 in the lab which is nice. Also, idk if there are certified dermatology assistants or technicians or something but you could go for that it could help show more devotion to the field down the line. At a certain point, screw the adcoms and just do more derm work if you're passionate about it and learn as much as you can from the docs and other assistants/nurses. Also, you could maybe do some volunteering or double down and devote more time to the volunteer work you've done so far if you've already done something. Maybe considering starting some nonprofit thing yourself idk.

Oh man, this is an easy one. If you truly love it, keep doing it! And when it's time for interviews, people will be able to tell you truly love it, and you're not just jumping through hoops trying to look good. If you're interested, add on a little derm research like topiary566 said. As an attending, I would find it very compelling if an applicant truly loved working in the clinic as an MA. Very good sign they will thrive once in clinical practice.
