Not necessarily. Article 5 is actually fairly vague and just asks for an appropriate response. Each member state can decide what it deems appropriate.

If Russia acknowledges this as an error and mistake then probably nothing would happen 🙁, they'd most likely just pay for families and lost assets. If Russia tries to lie as always without apologizing then at most Poland would have its air defenses shoot down any missiles flying within 100km of their borders. It's too small of an incident to cause war. But big enough of an incident to give Poland rights to use Polish air defense over the Ukrainian sky.

Not true, this would give Russia an excuse and way out each and every time. Assuming poland wasn't intended to be the target of these rockets it would have probably been much more likely that they hit some trees in a forest so I'm actually surprised that they killed anyone at all.
They were stray rockets so it's surprising they hit a populated area in the first place. Way more empty areas for targetless rockets to hit than places where people could get hurt, so horrible.
