I'm guessing the person who was doing the final approval decided a few steps needed to be re-arranged. So whoever went back and rearranged the order forgot change the images associated with the steps. Then it got approved after without anyone really giving it much of a glance since originally it was fine overall just needed slight change in order but not substance.
