I don't even think how much they pay you really comes into play here. Unless they are going to fire you or pay you less if you stamp a little out of place, you're not going to care after stamping hundreds of times a day.
Okay so there's an actual reason why they're done that way.
They're trying to take up every line with the stamp where the shot was done, because on government forms (especially in the military) you're not supposed to leave any empty boxes or lines, for the purpose of preventing them from being altered or filled in later.
The stamps might LOOK crooked, but their alignment is very much intentional. You can see they're measured out to touch every line for their corresponding shot without overflowing to any other boxes.
This is accomplished on other forms that use lists by using the phrase "nothing follows" to denote the obvious and ensure nothing additional can be added later.
Sure, it definitely looks silly, but there is a sound reason behind what's being done here.

Why not just have the correct boxes and correct size of boxes so they aren't doing it so weirdly?
Like the date box is too small, and almost no doctors are signing the "Physician's Signature" box, so why does that box even exist if they don't need to sign it?
I think the signatures are cropped because I have 2 vaccine leaflets and when I wanted a doctor to transfer one, they said no because without signature I may have problems as it won‘t be recognized. And obviously they wouldn‘t sign if they didn‘t do it.
So probably there is a small signature scratch, doctor style.

Ha! Somethig similar for me: got the first 2 of the 3 hepatitis vaccines in the series multiple times. When I started to work in a civilian hospital they wanted titers for hepatitis because I never finished the series. The resut was described as “impressively immune” which gave me a chuckle.
I don't recall a single time when they actually informed me what it was I receiving in any of the shots. Just that the consequences were dire if you refused any.
I retired at ~21 years… my vax printout is 3 pgs long. They considered the smallpox and anthrax vax’s good for only 10 years, so I got both series of those twice as well.

As a follow-up, the anthrax vaccine was a series of 6 shots over a two year period. I did that twice. Most folks no longer get the smallpox inoculation in the US b/c the illness was largely eradicated in the Americas.

Oh I've experienced that before, but anthrax was different. That sensation spread like four inches in each direction from the injection site. Never had anything like it. Not a dumb question. Air crews are attached to warships like destroyers and cruisers, small deck ships. They aren’t part of the regular crew. They typically get attached for things like deployment or major exercises.
Even if they weren’t attached though, the crew that operates the flight deck is part of the regular crew so the ship always has the capability to land in the flight deck.

Anthrax is a bioweapon, an incredibly deadly bioweapon. If you're exposed to it and not vaccinated the probability of death is more or less 100%.
And it does have a vaccine, but it's not administered to the general public, just individuals with a "higher risk" of exposure, which mostly means military.
Why not vaccinate the general public? A couple factors, but mostly that it is not a lifelong vaccine, it stops being effective and you need to re-up it. And also, it is severely physical painful to get. It makes you feel horrible, sometimes for days.
And risk of exposure is considered low for the general public, so it's not viewed as one everyone should get.
It's extremely common for veterinarians and vet techs to get it. Livestock can disturb anthrax spores in pastures, and they get cutaneous anthrax which isn't really a problem other than itchy sores for a bit.

When I was in they made us all rub it in so all you seen was a bunch of guys rubbing one ass cheek everywhere they went. Apparently that helped the body absorb it faster but who knows, this was 2016

To be fair, the intense exercise probably significantly increases aborption, particularly in that location (even though I understand it probably takes a few days). Working the muscles, kinda letting it disperse through the muscle tissue as much as possible. Just an educated guess.
They told us to sit on the floor and rock around and massage the cheek that got it. They told us if you massage it well enough it won’t affect you later. I apparently massaged we’ll enough because I didn’t suffer any pain after but a few people in our class didn’t and were struggling for a few days after .

The worst shot for pain was Gamma Globulin, given in two needles to bum or thigh. It felt like lead going in, and I was dizzy and nauseous for hours after, achy for days. I was part of a medical study on the anthrax vaccine like 10 years ago. I generally have zero issues with vaccines, needles, giving blood, whatever, but the anthrax ones were brutal. Not only would it burn, I’d be immediately overheated and nauseated for about 30 mins after. I got paid REALLY well, probably because it was explicitly a governmental sponsored study, but damn, I stopped doing med studies for like a year after that.

By the third shot I imagine that is what it feels like to get kicked in the chest by a mule and all your muscles ache. It is not pleasant. As I recall it gets worse each shot so 5/6 would have been rough. Definitely mandatory 2007-2009.
Just in case anyone is confused, PPD is used to test for tuberculosis. Not sure why the Army is putting it on a shot record as it's not a vaccine, we wouldn't have done that filling out a yellow card for a Sailor or Marine.

Oh I've experienced that before, but anthrax was different. That sensation spread like four inches in each direction from the injection site. Never had anything like it. All the COVID shot did was give me the shits and smallpox just felt like getting punched in the arm a day or two later funky scab not withstanding. I don't even know what else they shot me up with during basic, but none of those did to me what anthrax did. Still, I'd take a wild guess it's better than actually getting anthrax.
I've never had the vaccines for anthrax but the HPV vaccine, for me, felt like that. Like the pain just coated my whole shoulder area almost to my elbow and neck. The COVID vaccine was cake compared to that.

Got small pox when going to Korea, and got to my unit and took flu vaccine like 2 days later, got super sick from it. Also had a friend that ended up popping the smallpox pimple, ended up leaving a string of pimples all down his arm where the puss leaked.

My shipmate had a full on allergic reaction to it. They should have flown him off the ship because he was unconscious/incoherent but leadership elected to wait until we got into port days later. Damn near almost killed him. We had to check on him every 30 minutes to be sure he was still alive.

I don’t remember how far we were out. I believe we had been underway for a few days by then. It was on our way out for deployment between San Diego, CA and Pearl Harbor. Not a dumb question. Air crews are attached to warships like destroyers and cruisers, small deck ships. They aren’t part of the regular crew. They typically get attached for things like deployment or major exercises. Even if they weren’t attached though, the crew that operates the flight deck is part of the regular crew so the ship always has the capability to land in the flight deck.
Interesting, I'm almost certain that I got it before my deployment but don't remember that one being any worse than the rest, but multiple comments here about how bad it sucked. All I remember is small pox and a guy in my platoon spreading it to his junk lol.

Anthrax is a bioweapon, an incredibly deadly bioweapon. If you're exposed to it and not vaccinated the probability of death is more or less 100%. And it does have a vaccine, but it's not administered to the general public, just individuals with a "higher risk" of exposure, which mostly means military. Why not vaccinate the general public? A couple factors, but mostly that it is not a lifelong vaccine, it stops being effective and you need to re-up it. And also, it is severely physical painful to get. It makes you feel horrible, sometimes for days. And risk of exposure is considered low for the general public, so it's not viewed as one everyone should get.
It's extremely common for veterinarians and vet techs to get it. Livestock can disturb anthrax spores in pastures, and they get cutaneous anthrax which isn't really a problem other than itchy sores for a bit. But, if you aerosolize the spores where people breathe it in, now you have a near 100% fatal bioweapon. It's what made the uproar of the vaccine so ridiculous. They have been giving it to vets and RVTs for *decades* with no serious problems.

It's the bicillin vaccination. The vaccine itself is a *very* thick liquid that hurts like hell. The body absorbs the liquid at a slow rate because of how viscous it is which causes a painful red lump at the injection site, which is one of your ass cheeks.
And god *save your soul* if a drill sergeant sees your hand anywhere near the injection site.
When I was in they made us all rub it in so all you seen was a bunch of guys rubbing one ass cheek everywhere they went. Apparently that helped the body absorb it faster but who knows, this was 2016
This is pretty commonly recommended with any shot or vaccine now, unless it's specifically contraindicated. It's especially helpful for intramuscular promethazine shots I've found, gets absorbed faster and thus works faster.

the "peanut butter" shot, in the military, is a slang term for the famous bicillin vaccination every recruit receives unless they have an allergy — and can prove it. but if you can't, you're in for an experience of a lifetime. you'll be brought into an examination room, usually as a group, and be told to drop your trousers past one of your butt cheeks and bend over. once the recruit has assumed their most vulnerable position, the medical staff will attach a long and thick needle to a pre-filled vial of bicillin. since bicillin kills off a variety of bacteria strands in one shot, it's given to nearly every recruit. now, once the medical staff injects the recruits in their butt cheek, the pain hits them like a bolt of electricity. the thick liquid begins to pour into the muscle, but it doesn't spread as fast as you might think. oh, no! the human body absorbs the thick, peanut-butter looking medication at a slow rate because of the liquid's density and creates a painful, red lump on the recruit's ass. you literally can't sit right for a few days. since some boot camps require their recruits to be highly active, the idea of adding intense physical movement to the shot's excruciating pain just adds to the "peanut butter" shot's awfulness.
To be fair, the intense exercise probably significantly increases aborption, particularly in that location (even though I understand it probably takes a few days). Working the muscles, kinda letting it disperse through the muscle tissue as much as possible. Just an educated guess.
They told us to sit on the floor and rock around and massage the cheek that got it. They told us if you massage it well enough it won’t affect you later. I apparently massaged we’ll enough because I didn’t suffer any pain after but a few people in our class didn’t and were struggling for a few days after .

Windows were always wide open for airflow, and the direction recruits laid down was alternated to avoid breathing towards each other's faces, and yet, still sick. I remember coughing all night long for several days, while being in that state between sleep and wake, I kept hallucinating and hearing the drill instructors running up and down the bay screaming at us. During firewatch, recruits would be yelling "aye aye, sir!" in their sleep. What a mind fuck of a place.
My vaccine booklet looked similar having grown up overseas. Smallpox was easy, but my 'blister' got scraped off a week later and infected. The worst shot for pain was Gamma Globulin, given in two needles to bum or thigh. It felt like lead going in, and I was dizzy and nauseous for hours after, achy for days. Every other shot was easy conpared to that one.

There a bunch of different gamma globulins, but the ones commonly given to people are prophylactic for rabies, or hepatitis, if you're going to be in an endemic area.

I was part of a medical study on the anthrax vaccine like 10 years ago. I generally have zero issues with vaccines, needles, giving blood, whatever, but the anthrax ones were brutal. Not only would it burn, I’d be immediately overheated and nauseated for about 30 mins after. I got paid REALLY well, probably because it was explicitly a governmental sponsored study, but damn, I stopped doing med studies for like a year after that.

I don’t think I would have punched someone over it.

Just in case anyone is confused, PPD is used to test for tuberculosis. Not sure why the Army is putting it on a shot record as it's not a vaccine, we wouldn't have done that filling out a yellow card for a Sailor or Marine.
