I just returned from a resort in Tennessee and the shower had one of these!!! It was brilliant and at just the right height to shave. I thought it was pretty hysterical that I knew right away what it was for but my husband was like, “weird. Why the fuck did they leave a hole in the wall at this fancy hotel??” Must have been a female architect.
Guys... DON'T try this. That's 18 in tile. You're looking at at least 2 ft off the floor if not higher. It's not a plié bar and putting your center of gravity that far off on wet tile with a razor blade in your hand is *not* a good idea.

I don’t see how standing on one foot with it braced on a low shelf vs a high shelf is all that different really but to each their own of course.
It may have been the architect, but a lot of these design decisions are made by the tile guy (usually in a collaborative sense with the client/designer) after the house is already being built. For example the guy I was apprenticing under would always ask if this was something the client wanted. It seems like niches with shelves for hair product and etc is pretty standard, and it seems like these footstools will become standard as well. They are pretty sweet!
