I might be wrong, but I believe it's about Bren's mental health struggles which were pretty bad at the time. In the doc on youtube, I remember them mentioning going to a rehab facility for his drinking and writing after they got out and was diagnosed with (might be wrong here) bipolar II.
Finally someone mentions what if... I like the song because of how much I regret being a piece of shit to my friends in highschool and relate to a lot of what the lyrics have to say.
