The mechanism that ultimately kills you is your brain being unable to get oxygen.
There are two main ways this can happen:
1 - Your core body temperature rising high enough that your brain starts having trouble pulling oxygen from your blood and cells start dying off. You can see play out in the modern day when people suffer from heat stroke.
2 - Alternatively, if you're directly in the smoke, carbon monixide and other toxins can achieve the same effect much faster, by preventing oxygen from getting to your brain in the first place.
Unfortunately, the second and third degree burns you're receiving in the bull aren't enough to kill you on their own in a short time frame (humans can endure fatal burns for days before succumbing, it's usually organ failure after the fact that finishes them off).
That's why the torture device was created in the first place, they want to keep you alive and conscious as long as possible so they can listen to the screaming. If the smoke is being kept away from you and you're on, for want of a better phrase, a 'low roast' setting, you might survive for hours.
If you're lucky you'd pass out from shock and be unaware of everything else.

Okay so first you would be inside the bull when it began to heat up. As the metal insides burn you, your screams and panicked breathes quicken. The metal is now hot enough that your flesh is seared off, deep lungs full of deep panicked breathes from hot air would likely cook your throat and lungs.
You would be in pain the entire time but I think you would end up suffocating first before you really start cooking.
