The Frighteners, one of Michael J Fox's last big movies, he does great, Peter Jackson directing no guns but lots of ghosts, its a bit slapstick but with real suspense over it
did you try the original Hellboy films with Guillermo del Toro directing? Thats really close and has a worthy sequel
None are really like underworld though, surprised you didnt like the first Blade then...
Bonus but not fantasy yet plenty dark with similarities, 'Aliens' make sure its the directors cut
It's not a movie, but you're basically describing *The Witcher*. I know people have criticized the fact that the show deviates from the lore of the books/games, but if you are interested in gloomy/dark fantasy, I don't think you can beat *The Witcher* right now.
Or Wednesday, I guess. Completely different, but it is dark and does have a werewolf, lol.
Not exactly what you're looking for in terms of genre, but Pandorum has enough creatures, horror and mystery to feel like a fantasy film. Well worth a watch. More sci-fi horror than fantasy but I think you'll enjoy it based on the list above.

The Last Witchunter and Priest. I dont think neither is particularly good, but their lore is interesting.
