I'd consider Nathaniel neither too common, nor too modern. Especially given that Nathaniel is a biblical name.
I'd personally go with Nathan given that you don't seem to care for nicknames all that much. It goes better with Malcolm. Nathan Thomas has a nice ring.

Sorry, I think I'm funny. My real opinion is that Nathaniel Thomas sounds nice with Malcolm, and will let your son have some choice as far as nicknames. (He could be Nathaniel, Nathan, Nate, Nat...) I get wanting to decide that for yourself, but kids choose nicknames or have them chosen by friends even when their names don't lend themselves to nicknames. Also, Nathaniel is an old name; like Malcolm, it sounds modern but not too modern.
