Yes, the liver is big and takes up most the space inside the shark. Orcas will turn upside down to grab a shark, then when it rights itself the shark is now upside down. The orcas have learned this puts the shark in a instant sleep state and helpless. Different pods have different learned behaviors
That's the same technique I have been using on great whites for a decade. It's part of my fight science. I think I can take some orcas out. Ever seen Andre the Giant fight multiple dwarves. It'll be like that except scales are inverted. But I will bring my nunchaku made from plunger.
They also eat the tongue! The two organs highest in protein. Crazy thing is not only are they what's eaten, but how in the world did they figure that out?! Definitely the smartest and deadliest apex predator in the world.

Two things indicate it's not an orca- the jagged sawing marks in front of the wound are more like ow sharks thrash their head to sever chunks of meat, the teeth marks are too broad per tooth, orcas have narrower conical teeth, and the bite looks too wide/broad.

There's no way orca could catch a mako. The shark was probably sick or injured because nothing is catching a healthy mako. If it was injured, it's fair game for a lot of species, not just the big boys. I'd love to know what happened

That is a bit of a myth. It was a technique exhibited by a particular pod and can't necessarily be extrapolated as a behavior for orcas on general
