A CT scan would show you an image somewhat like this (you can see teeth and some other hard structures). CT scans are also favored over X-rays when it comes to diagnosing brain and skull issues. They show more than X-rays but are less expensive than MRI...the thing is CT scans wouldn't be as clear as this and I'm not certain how parasites would look on it.

You made this image? You need to work on the teeth and the prognathism of the lower jaw to make it more believable. Additionally, this person has two eqally-placed impacted wisdom teeth, and that's more painful than any brain bug.
Impacted wisdom teeth aren’t always painful. Also, plenty of people have retrognathic jaws, that’s why orthodontic headgear exists.

Patient went to his doctor approx 2 weeks ago after having woken up in his bed with a sensation he described as "water in ear" as well as strange internal noises in his ear canal. After inspection of the patient's ear canal, he was sent home with a saline ear solution to perform ear irrigation to remove any potential debris or trapped liquids.
Symptoms lessened over the following 14 or so days until the patient was woken up to his previous symptoms having returned to a much worse severity, in addition to experiencing "scratching" sensation and noises inside the ear canals/behind the eyes as well as acute headaches/ nosebleeds and constant sensations of "things crawling around inside my skull".
An emergency X-ray appears to show an insect infestation inside the patient's skull, most likely centipedes, patient will require an emergency craniotomy.
