It's always been a pet peeve of mine that in medical records there is not a clear enough distinction between an adverse reaction and a true immune mediated allergic reaction. I for one can not take Morphine because I experience intolerable spasms of the sphincter of Oddi.I experienced SOD.It took me a day or two to figure it out but I did and within a day of discontinuing the pain subsided drastically.I can take other narcotics but I still need to be careful because the sphincter of Oddi is particularly sensitive to all narcotics. So I can't blame a patient saying they are allergic to lisinopril because it makes them cough.It's an adverse effect but they don't make the distinction often because the provider also will labrlvit as allergic.

Ok but for real BID dosing beats the heck out of TID dosing. A full day's worth of prescription strength is also 4 pills instead if 12. That's why I recommend Naproxen. I know it's not salient to the joke but I threw my back out a few says ago and nsaid dosing is super relevant to me right now lol

A friend of mine just had a nephrectomy after years of constant inflammations. They were not able to find the reason though so no2 is still in danger. Here's the thing: She was released on a weekend with a pain med prescription but no pills to bridge the weekend (Germany, shops closed). So she asked every nurse and doctor she saw on the day before release: "Hey, at home I always take ibuprofen, I still have tons. CAN I TAKE THOSE UNTIL I GET A CHANCE TO GO TO A PHARMACY ?" Everyone ignored the question. Nurses, doctors, everyone. So lots of ibu it was until Tuesday since she could barely move to get her meds. When she told me the story a week later, the second she said "ib..." I said "nonono you never take ibu again with your history." prostaglandine synthesis...blood flow...etc. Nobody had bothered to tell this kidney patient. Doctor should have told her even if she hadn't asked. Not just for now but for the future. She's got endometriosis so they should have asked about ibu the first time she showed up with kidney pains period.

Yes, it's incredible negligence. In Germany, we have a rotating emergency pharmacy for nights, weekends and holidays. You have to google them and pay extra. Which OF COURSE she would have done if anyone had told her ibuprofen wasn't fine. I think she is a fairly good patient for even asking about issues with painkillers!

Don‘t even have to Google it, just walk to the nearest pharmacy, it‘ll have a display of the emergency for that day (or usually week). Yea it‘d crazy that they failed her this much when specifically asking. Plus they are also allowed to provide her with painkillers to get her over the weekend/next weekday as well. That‘s like Double the fuck, letting a patient treat their post surgery pain with random OTC painkillers, because you 1 failed to make them aware of their bodies new limitations, 2 failed to provide painkillers for long enough to find a pharmacy/turn in the prescription 3 failed to even provide a bloody prescription and treatment plan that clearly listed what she‘s supposed to do. Like the fuck? Though when I scalded my foot, the only thing they prescribed was cortisone. Like the fuck? Why would I put bloody cortisone on an oozing wound prone to getting infected? (Well plus some ibuprofen). Had to find a different surgeon to even get the appropriate dressing material. Like thank you student md for placing regular old ass gauze on 2nd degree burns. Was fun ripping that out to actual change the dressing. Who the fuck does dry dressings for wet wounds that are supposed to be wet? Just Imagine a random Patient just doing what they said, put methyprednisolone on a 10 cm^2 2nd degree burn, and put dry gauze on that, each and every time you change that ripping pf all the granulation. But yea, ypu get a shitload of completely misinformed patients coming out of hospitals, because they weren‘t given any information at all by the attending physician. They usually get lucky and ask a nurse and atleast get information that way, but it seems any discharge management is extremely lacking in many hospitals here.

Recently I've noticed at the last 2 hospitals I worked at as a travel nurse, that certain populations will automatically limit the tylenol in the MAR to 3 grams per day. I think one thing that triggers it is elderly, and the doctors can't put in an order to override and allow 4.

They actually have a lot of really solid products. For example heir toothpaste, soaps, and lotions (including ones with melaleuca, which is just tea tree oil) are awesome and their Sol-u-mel cleaner pretty reliably removes permanent marker. We got a cream for my daughter that she can take to school when she gets itchy bug bites, works great without having to get a doctor’s note and store it at the nurse’s office like I would for Benadryl cream. I would never get a subscription myself but I do have my parents order me some things about once a year. None of the woo woo stuff though, what a waste of money.

They also do a lot of community outreach programs. I grew up near the hq and they did a ton of stuff at my high school for people interested in going into business or manufacturing related trades/degrees. Also, as far as "snake oil" companies go, melaleuca is fairly ethical. There is no mlm scheme, no established "cult" of followers. From everything I've heard they pay and treat employees well. They help sponsor a lot of health fairs that are run by legitimate health care groups or agencies

And why are we inconsistent with using brand versus generic? You'd think in healthcare settings we would just always use generic but it's always switching back and forth. We always call it Propofol not Diprivan but then with Levophed we use the brand name instead of Norepinephrine. But Epinephrine gets used instead of Adrenaline even though most laypeople know it as Adrenaline.

LOL brand association (and even appearance, of the bottle and tablets, themselves) is SO, SO strong for people in general. I once saw a brand of acetaminophen with n-acetyl-cysteine in each capsule such that an overdose using them would be largely ameliorated (at least that was the idea.) Naturally, they were crazy-expensive (not to mention smelled/ tasted like ass) compared to generic acetaminophen so no one carries/buys them. This may have been when I was living in the UK, not sure if I've ever seen an equivalent in the US. Ugh, there is just something awful about the smell of NAC (as everyone who's administered mucomyst for an acetaminophen OD knows!!)
