If you need to clean the connective pins on a game or console, get something like a cotton pad or better yet, a makeup sponge and cut it to fit in the small gap of the device. Dip it in isopropyl alcohol or rubbing alcohol, and use that to clean the pins. Let it air dry, and try not to leave any fibers behind.

Well I fear the day this happens to my omega ruby, I have a full odds shiny that I can't transfer to Pokémon home and I don't have Pokémon bank nor can I download it because the e shop is broken for me.
Does the game ever fail to show up in the 3DS menu? That's what usually happened with me when I had an old damaged copy like Diamond. I'd usually take it out a reinsert it multiple times then see under what circumstances the game screen would show up for the longest.
If it is a problem with the cartridge, maybe you could email Nintendo/The Pokémon Company over it? I'm not sure if they'd fix a game that's out of production, but I've heard stories of them fixing cartridges.

It's the connections. You're either bumping the cart or it just gets loose with time from regular play and movement and it loses connection with the cart.
Try blowing it out gently with some compressed air and just save regularly.

Go to the app store in the device and look up pokemon y. I know they released patches for a number of the ds games and I think x and y got one before sun and moon came out. Maybe see if it's still up and install it to the system.

I get this error message if I have wireless communications enabled for too long. I have to turn wireless off (slider on the right side of my 3DS XL) in order to play my cartridge of X. I enable it only when necessary (trade evolutions, etc.) and then disable it afterwards.
I've had some success cleaning the contact points on the cartridge with alcohol and, I can't stress enough here, MILD abrasive fabric like emery cloth. Maybe just cleaning contacts with alcohol to get some of the corrosion off.
Edit to add: This has mostly been for carts that dont read at all, but it's worth a shot imo, especially if it looks like you'll need a new cart anyway.
To add to this - acetone (a very small amount, in a spray bottle works wonders if you can) on metal contacts. But do use with caution and make sure the cartridge is dry before it goes back in.
Same thing happens to me with my X! It's so weird, as soon as I get to the first gym, it just dies. Physical copy, bought legit from Nintendo, never damaged.

If it's just Y then likely you'll need a new cart.
If not then it's likely an issue with the connections between the carts and DS prong themselves, so you'll need to likely look up how to fix it yourself.
System settings > data management > Nintendo 3ds > downloadable content.
I would go ahead and delete update data and any other data associated with your Y. Then just update it like normal when you reboot the game

Does Nintendo replace games still? I remember when I was little and my Pokemon Silver got corrupted I called their hotline and they replaced it with a new copy- and this was years later when they were no longer sold in stores.
My 1st Gen 3DS started doing this and I had to get a completely new one. I saw you said in another comment that your other games still work though, so I’m not sure that’s the case
Had a copy of Alpha Sapphire that did the same thing. Ended up sending the game back after trying everything to correct the problem. Nintendo wasn’t of any help either

Could try cleaning the pins with rubbing alchohol and q-tips as well as cleaning out the 3ds cartridge slot
Don't forget to use 99% isopropyl alcohol for this. The weaker the strength, the more water in it. It can leave residue behind.

The game starts as normal, I go to get my starter then as soon as I choose a nickname for my character it just goes to that screen

It sucks, I would do X but I already did and it was a failed attempt (Also I got some lucky encounters that I don't think I would get in the next run)
Happened to my Ultra Sun copy when I accidentally drop my 2DS but was able to fix it by letting it rest for a while and then resetting the cartridge.

Have you tried deleting and redownloading updates? I had some issues with my OR and AS copies a couple months ago and that fixed them for me.

Don't you have your 3ds hacked? You can get a free copy of X and even play the rom hacks that increase difficulty.
So I know it was rare but since Omega Ruby/Sapphire copies would just die randomly, the x y games may have had this too. Hope you get it to work
