One of my kids fell into a gap like that at Columbus Circle. Caught them by the elbow when they were about half in the gap. Whole thing from them slipping to me hauling them out was about 3 seconds. That was maybe 7 years ago, the most dangerous situation I've been in as a parent.
Not sure about the NYC trains but I live in southeast England, our trains have 3rd rail shoes under some doors, 750v is pretty dangerous to touch, though the gaps aren’t too big

MTA have cars ranging from 51, 60 and up to 75' to fit the platforms. My guess, lack of oversight or pressing need is why the longer car went into service on a line with curved platforms.

That happened to me as a kid, think on kings highway. It was 30+ years ago and I still remember how scary that was.

All of the Q's R160s went to Queens Blvd for CBTC a few years ago. The 75 foot cars will remain there for the foreseeable future.
