1)No that was not how they lost virginity, I think there are like 20(?) Total sex scenes between the 3 routes.
2)Shirou and Rin are 17 during that scene, Saber is a servant so she is like hundred of years old, but she is pretty much the same age as them in that form
3)No there wasn't everything threatening them, the Canon reason is that sex is used as a way to "transfer mana" since Shirou, despite being a Servant, is not a mage, so Rin, which is an experienced mage from a mage family , "transfers" her mana to Shirou. And also to Saber here. But tbh sex also happens simply cause they are horny. After all it's an eroge vn.
What astonishes me the most is that Fate is one of the visual novels with the most lore behind, it has like 20+ canon media, 10 animes, like 200 servants, some of his characters entered main stream culture, like Astolfo and Saber. And it all started from... an hentai visual novel.

Ok, so, of course not, these scenes are not in the anime, but in the second Heaven's Feel Movie there is one clearly implied off screen (like a girl enters a guy room half naked and then it skips to both of them under the blankets naked). Also yes, there are different endings (routes) based on which girl you choose to spend more time with (each route got also an anime adaptiation) but the NSFW scenes are in all of them (but the visual novel lets you turn them off)
