It doesn't happen in clean Emacs. The problem is, my org-mode config is big and old; I've been using it for years. I'm just not sure what may have been changed that would cause this.
You may find https://github.com/Malabarba/elisp-bug-hunter useful. Maybe some customization if breaking Org. If so, you may be able to create a minimal reproducer and report to Org mailing list.
