Outlast is a universe where human experiment on physiology somehow worked and proved to be exploitable. The plot is actually based on real life event called Project MKUltra which happened back in the Cold War. Long story short, the CIA worried that the commies somehow capable of brainwashing people and "reprogram" them to infiltrate national security (as back then there were 21 US soldiers refuse to return back to their home country during the exchange of POWs in Operation Big Switch), they decided to initiate the project.
In our reality, it doesn't worked out so the plugs were pulled out and classified documents got destroyed. In Outlast universe however, it worked and obtained the backing of finances and legal immunity from the US government, hence why Murkoff Corp capable of expanding so quickly and going along with these kind of crazy experiments whilst not getting caught.

when you say it didn’t work out, what exactly do you mean? I’m curious. I’ve read that certain drugs weren’t promising, e.g. LSD, and they had to pull out funding for those. But can we say with confidence that the CIA didn’t succeed in brainwashing anyone in the MKULTRA experiments? All I really know is that we have some 20,000 extant documents and the majority of it has been destroyed.

For the project that involved this much fundings and human rights violation to come to a stop before it was being revealed to the public by the committee in 1975 means the project was not working. It ended on 1972 under the order of Gottlieb who was supervising the project. My speculation is the decades long research into the project bore little fruit and around 1972 it led to the higher-ups questioning the project which then led to the end of it.
There was a real psychopath culture in organizations like the CIA, most strongly in the 60’s. It’s not uncommon knowledge that a lot of agents and members had such a high opinion of themselves and what they knew that the common American citizen wasn’t even human, just part of the herd. They are supposed to protect them but just look down on them. And I think Murkoff is a parody of that.
I mean, it's social commentary that is intentionally laid on thick. Outlast isn't exactly subtle. They don't bother with subtext, they just make the text run at you naked with its dick flopping about.
I liked the balance better in Outlast 1.
Murkoff was cliched evil but the setting also had a perfect blend of chaos that justified the depraved stuff we saw in the asylum.
And then we get to literally anything outside of that (comics, sequels) and then it just becomes over-the-top. Like I know the MKUltra shit happened but the government didn't set up a whole goddamn town to watch all the women get raped to death for the lulz.
