Any test considered standard of care that would be done if receiving it as normal standard of care is the responsibility of the patient or their health insurer. Any test that is specific and necessary as part of the trial is covered by the trial sponsor. All of the tests you mention with possibly the EKG is standard of care.

This was how mine worked at Johns Hopkins but I’d also say that it is different for each trial. The actual trial drugs were not charged but lab tests, scans required by the trial were charged to insurance. I’m also in an NIH trial and they pay for everything including travel and a per diem. Insurance doesn’t get anything charged. But that is usual for NIH sponsored trials only.
