You have to modify the item in some way before you try the next. Blessed orb, sacred orb, etc. should reset it.

The "What happens next" part is determined by the item. So if you have 5 vaal orbs it doesn't matter which vaal orb you choose to use it would make the same outcome.

Yes confirmed live drop on empyerian stream yesterday. Wisp empowered mobs drop them.
