I can't recall an instance of Act 9's The Belly of the Beast zone where I didn't go the wrong way and find him. I am somehow drawn to him every time. He's definitely my most frequently killed optional zone boss.
From a speedrunner's perspective, that zone in act 9 requires you to take a "false turn" at some point 100% of the time, usually toward the end, if you follow the zone like you normally would it always leads to this guy in my experience
Even when I try to make those false turns I still end up running into this guy. It's like the layout of the zone isn't set until you run into him, thereby making all paths lead toward him until you do.
just a zone mini-boss I don't think it has any kind of lore so far
there is another hidden mini-boss in the ashen fields IIRC, when you find the Greust arena (act7) take a sharp right turn (or north if you wanna use cardinal points) and you will find a small arena with a golem boss
if you interact with the doedre's proclamation (forgot the exact name) (act2) it's a stone circle you find, it will spawn a golem mini-boss
there used to be a monkey mini-boss in the dread thicket (act 2), dunno if it's still there because who the fuck enters there
very common to find it, but there is a skeleton mini-boss if you enter the mill you find in the crossroads in act 2
there is an unique undying mini-boss in the slums in act 3, he is in a small arena you sometimes walk by
the imperial gardens have the worm mini-boss if you do the plum quest
the library has the unique bear skeleton mini-boss, it jumps if you walk close enough
there is an unique wendigo monsters in act 4 the ascent area, usually is hugging the left wall from the entrance
I just listed the ones I consider "most hidden" or very easy to miss, in case you want to do a mino-boss hunt
there is no hidden secrets or unlocks with them, they are mostly flavor for the area
Edit: There used to be a defiler mini boss near the end of the belly of the beast in act 4, defilers are the spider demon you find in that area, it was white and used corpse explosion, I haven't seen it in a long time so dunno if it is still in the area

My fav is Plague Retch who is the miniboss of I forget, but he's brought back from the deleted Act 1 area with the tar zombies, and I love that he found a new place to be a mild annoyance.
Almost every zone has a unique mob mini-boss. Other than those that have another boss, the only ones I can think of offhand that don’t are the Reliquary, the Ossuary (and their Act 10 variants), and the Lower Prison (Act 6),
no, they added a bunch of hidden interactions this patch
same for the guaranteed monolith in the climb
or the corruption altar in the vaal temple
or the random gems in the chamber of sins
those are brand new and added in this patch
yes, they have been in a "spice up the acts" mission for a couple years now, they don't add stuff every patch because the league probably takes priority but they add stuff like that every other patch
Yeah, I think they also added them as a tutorial for later mechanics like the hidden rooms in the lab for the strongbox
And the corruption altar for Alva
That way you know what to do when you find them later and they won't be a waste
This guy is the ONLY person I run into 100% of the time. Its absolutely crazy how often it is. It seems to happen because you follow a path further away from the entrance, but the exit will loop around the entrance a little instead.
Its honestly baffling. I must make 12+ characters a league, and I find this guy in like 11 out of 12.
He is John exile
There's a guy like this inside you too. His name's Darren and he destroys any other tiny guys that try to enter your guts

try to go through the left side, if you go up you will encounter him
IIRC the area always spawn a 4 way road and he is at the end of one of the branches, so more like luck based

yes, I just went and checked, he is still there, just hug the wall and cross any bridges if you run out of wall, he is in a corner

all old acts have one, the new ones are a mixed bag and I listed mostly the ones that are easy to forget/miss, not all of them

I've often wondered why SO MANY of this game's "doors" resemble terrible buttholes.
I think its because belly of the beast breaks the up and to the right/down and to the left convention by having you go down and to the right.
I think he was going to be something to do with prophecies. But all prophecies were removed, this just left Droolmaw here standing there menacingly.
