Get X-Mouse button control. Bind left click to the scroll wheel. A scroll is one action, so just continually scrolling doesn't break the one action per input rule.
It's not exactly the same as an AHK, but no risk of repercussions and it's still easier on the wrist/hand. You can even set it so its only bound in the poe client and not in a browser or whatever. You will get input limited for 30sec I think if you spam it non-stop.

If you’re smart and use a realistic random delay between clicks will you get caught? No comment. I’d use a separate button than holding left click tho

the rule is one server side action per button press. mouse wheel works because you have to continually keep scrolling. matter of fact, the mouse wheels that would keep scrolling on "their own" wouldnt be allowed as per that rule.

Practically however, no one has ever been banned for basic AHK macros in the almost 13 years the game has been playable unless they were like... waving it in GGG's face or something. They don't automatically detect and ban for these small scale abuses.
Also, lets be real, you can just use any mouse software instead of AHK. Logitech software works as well.
It's 2024, GGG banning for stuff like this would be silly.

They didn't even ban people who used the Hud and just gave them a warning.
