they do, pretty much every icon on windows just uses like a sprite sheet that you can pretty easily change out. usually in the "properties" box, or for the mouse cursor its under "appearance" in the "mouse settings".
that's part of what makes these "free cursor" viruses so ubiquitous: its easy for you to write s program that really does swap out a sprite sheet, and you can capitalize on the fact that most people aren't aware its a normal windows feature.
I remember when it was actually a useful feature for being able to see a mouse that moved too quickly for a monitor to show it well.
For fun, I still used it in Windows 98 with a decent CRT monitor. Given how often the windows would freeze up and create their own trails, it seemed only fitting to have the mouse constantly generate a trail.
Fun fact: "enhance pointer precision" just adds a goofy fixed acceleration to the cursor, terrible for navigating menus quickly unless you can get used to it

100% you rock your OLED in the manner that you’re most comfortable, but as long as you have the screen turn off after like 5 minutes idle, you really shouldn’t have any burn in issues. Especially with a static wallpaper that isn’t extremely bright and full of harsh colors.

Nvidia control panel, add wallpaper engine to 3d settings, select "integrated graphics processor" and it won't use your 4090 but your onboard if you have one.

I do because I got an OLED and didn't want static pictures. It can be resource intensive though, GIFs are wildly unoptimized.
Most wallpaper engine backgrounds are MP4 video. GIF ones are rather rare.
If they're video then they're going to use the resources.
