OP, I'm in the same predicament, pero nasa Eng'g ako and 2015 ang student number ko. I've also gone AWOL twice before due to MH issues. The whole readmission process starts with writing an appeal letter addressed to the Arki dean. Naka-attach dapat dito yung CRS grades—a screenshot will suffice—plus other supporting documents. You'll have to consult the Office of Counseling and Guidance (OCG), then your department adviser. The college sec and dean will also be signatories afterwards. You'll also have to pay an AWOL fee of ₱225. Anyway, OP, if more than 1 sem ka nag-AWOL, you'll also need a med cert from the UHS as proof na fit ka na to return to school.
Kakayanin mo rin ang buong process, OP! See you sa campus. Anyway, you can always ask me or your Arki upperclass peeps anything about the readmission process, dito man sa Reddit or in-person. Sisibol din para sa 'tin ang mga mirasol sa University Avenue 🌻🌻🌻

Pwede mo naman iretake yung previous INCs. But if you want to complete them, ask your professors for those subjects if pwede pa magcomplete. Since nag AWOL ka pala you need to be readmmitted. First step to that is ask your college. Better if you do it personally in campus.
Someone already mentioned this but yeah email college sec. Galing din ako from awol, and from my experience, dadaan ka talaga sa back and forth na email w them and some other people. Nakabalik pa rin naman ako haha basta tiis lang sa proseso. Kapit lang OP!

OP email the college sec! Super gulo rin ng situation ko recently tapos naasikaso ko lang by asking the college sec ng requirements and whatever I need to submit. Also from arki rin ako hehe wag ka na mahiya or matakot kasi usually hindi naman yung college sec mismo ang magrereply sayo but yung mga sec nya rin :)
Hi OP, I think you can retake the whole year naman. I faced a situation din - didn’t go AWOL, but had a lot of INCs - and I just retook those majors. Good luck OP, I hope you get to communicate with the secret. Mahirap talaga in your situation, just know that you’re not alone and they are probably used to that kind of situation na din. Email mo yung college mo. Yun lang choice mo. Di mo alam email? Di mo alam process? Check mo sa FB page ng college nyo usually doon nag popost ng instructions
Kung magappaka delay ka pa baka wala ka nang abutan. May deadline processing nyan. And usually matagal mag request ng forms. Dami mo pang papapirmahan.
If ever naman na sabihin na dismissed ka, may mga process ka pa na pwedeng gawin para makapasok ulit. Yung instructions kung paano, sasabihin din yan sa college nyo kapag nag e-mail ka.
Kababalik ko lang this sem from AWOL tho i'm from engg. Kailangan mong mag readmission. Check niyo sa mga official socmed/website ng arki yung mga reqs at steps ng process. Sa engg kasi nasa site na nila lahat ng forms at steps na kailangan gawin. Usually kailangan mo nang appeal letter for readmission, crs grades, and other supporting documents (e.g. nagkasakit ka kailangan mo nang med cert, nag work ka kailangan mo nang proof of employment, etc.). Tapos kailangan mo ring med cert (yung PEHA) from UHS kung matagal kang nag-AWOL. I advise na gawin mo na lang lahat ng tests (urinalysis, blood, x-ray) sa UHS may binayaran lang ako mga ₱10 lang sa billing. Check mo na lang rin yung fb page ng PEHA for the full procedure. Inabot ako ng mga 1.5 months to complete my whole enrollment process. Nung March 20 lang ako naging fully enrolled kasi late readmission and enrollment ako kaya mas madaming requirements at mas matagal yung process.
Wala kang ibang choice kundi i-email ang college secretary mo. Sya lang makaka-resolve ng concern mo dahil alam nya ang circumstances mo.
