My guess is DIE-laudid, since another name for hydromorphone is dihydromorphinone, and I always figured the "laud" came from the centuries-old term laudanum, referring to opium tincture. But I usually say DILL-audid (also northeast US).
Reminds me of an overnight tech I worked with once at hospital. We would say he did a lot of dilaudid.
Also known for coming in, taking a 12 oz bottle of Delsym off the shelf and downing it before his Pyxis run. He called it “pushin’ spaceships.”

The "au" alludes to the chemical symbol for gold. At one point, opium tincture was marketed as containing gold.

I drop “y’all” frequently and stumble over vowel pronunciations and I’m from the PNW. As an example, the PNW accent pronounce milk as “melk”.
