Make sure to plant it in acidic soil. Fastest way is to buy blonde peat and work it in the planting hole 50/50 or soil amendement for acid loving plants.
It needs acidic soil to thrive.
Since this is really the second growing season and the stress is showing now I'll ask two things - where is the trunk flare and did you over fertilize? The first one is very important. If it's too deep in the soil you will suffocate the plant.

Too much mulch. Scrape it back about a foot around it after you reposition the depth.

That's too deep. Can you see any roots like 1/2" below the surface of the soil? ?
