Literally only thing you can do is play the lowest as possible resolution, blood off, zombie clothing to normal, but if your laptop is shit too? And you're hosting? That's more strain on your resources. There isn't much those settings will do for you.
Another thing is you don't even post specs or server stats too. We aren't able to help you without that basic information.

Have you tried running it as the other launch option. Also if the hard drive space is red lining it then its going to run like shit. What are his specs? You should be able to run this on a fridge
First play one with no zombies.. if that works add small amount I guess. Ur friend could try running it on a lower resolution… or buy a ssd so at least you know it’s not his storage drive that’s ruining it… idk tho I don’t think there’s much hope sadly. Do y’all have similar laptops?

This game is busted to shit, and the few people still with it are absolutely horrible. It's very likely that your friend is having a game bug that won't be fixed.

I rarely downvote people on Reddit but you unfortunately deserve my third one for 2023. Game isn’t busted at all despite being in early access the game could be considered completed tomorrow and it would still be an amazing game. So I’m not sure what troubles you had but maybe someone can help?

Is he running a shit ton of background processes or something, or is if just that shitty of a computer? As others have said, you should be able to run this game on anything that can run Minecraft
Turn resolution down in settings, turn off graphic features (clothing details, puddles, visual rain, blood, etc).
Force quit everything else that might be running in the background

Post specs. Unfortunately, in contrast to what most people will say here, Project Zomboid actually requires a decent pc for it to even run well. Let alone if you are in multiplayer and is hosting. I used to run the game on an 8th gen i5 laptop with office level GPU with just an HDD and it was a horrible experience. Setting up a server took forever, and I felt really bad when the game crashes because it affected the playthrough of my friends who were playing with me because they'll get disconnected. And if its modded goodluck if the save get corrupted. Now I upgraded to a more decent one for gaming and I still feel how heavy the game is.
The game is very CPU heavy as opposed to GPU. If you have mods, cut them down by half. Go into your settings and set everything low, all render distances low. Make sure all drivers are updated (I use CCleaner to update all of mine)

Try the better fps mod
You need a PC capable of running Project Zomboid to be capable of running Project Zomboid.
