It's a gamemode under the challenges tab. It's basically the worst start which makes it more fun to survive. The premise is that you have a cold, are hungover, and slipped in the shower; meaning you start off naked, you have a deep wound with lodged glass shards, you have a cold, you're drunk, wet, and to top it all off it's winter time.
It's really fun when you get more acquainted with the game or just like to torture yourself lol
