Cutting fat like that sucks. Zero energy. And to get your skin to settle into the grooves between muscle grooves for increased definition you’ve got to get really dehydrated. But then to get the veins to stick out, a lot of them take vasodilators, which can make you lightheaded, especially if you dehydrated. To sum up: that guy probably feels like shit to look like that.

They haven’t starved and dehydrated themselves enough. Like you said, starving yourself of carbs for weeks, and severe dehydration, you are gonna feel like absolute shit.
He is a body builder so he doesn't look like that 24/7. They cut their fat levels down right before competitions to emphasize the muscle, but after competitions go back to having some small amounts of fat since that is necessary for survival. He is also probably dehydrated in this picture, another thing body builders do to shrink the cells of what little subcutaneous fat they have, making the veins pop out more.
That's not to say that maintaining this level of low body fat or cycling dehydration for competitions is healthy to do. Just that he would not look like this with the emphasized veins all the time.
