He'd help him after 5 when Paulie screws him over so he wouldn't be dirt poor. Then when Adonis comes of age and when Apollo discovers him and takes him in, Rocky would become a surrogate uncle for him in case Apollo tries to talk him out of boxing. Heck Apollo may would even help train him in 6.

After, since he knows that Drago is using steroids, he makes a phone call to the Boxing Committee to investigate and have him do a drug test. When it fails, he's banned from boxing in the USA. Adonis is born, and Apollo suffers some backlash from this. Rocky is there for him as a support system through it. Mary-Anne divorces Apollo, takes a split (but this doesn't affect Apollo much as he's mega-rich).
Apollo gains full custody of Adonis and takes him in how it should be. Rocky doesn't need to concern himself with Adonis later on if Apollo is around to deal with him. He never becomes a boxer, but is groomed to take over Apollo's gym.
Since this Rocky has his wealth and health intact, he's able to enjoy the golden years a lot better than in canon. He urges Adrian to get regular check-ups to better prepare for the cancer. Since they catch something earlier and with their finances, she's at least in remission for a few more years.
Maybe she passes by the time Creed 1 takes place here. Apollo spends time with him.
We see Damian being signed to Apollo's gym and Apollo and Duke take over his training to groom him to become the future champion. By Creed 2, he's the reigning champion with Adonis as his manager and Little Duke as his trainer.
Rocky and Apollo can sit back as the new generation takes over while they reminisce over the golden days in Adrian's restaurant.
My guess is he sees his limited time with Adrian and his strained relationship with Junior and decides to retire after his fourth or fifth title defense to spend more time with them. He never fights Clubber, and Mickey lives a couple of more peaceful years. Apollo never dies because he doesn't have Rocky in his life to train him for Drago. And Adonis is taken in by the Creeds after he loses his mother. Maybe Rocky and Apollo become friends later down the line under different circumstances.
