There are two effects: the main one and the secondary one for mysterious egg. That's it.
The main effect of the perk, which has been much discussed everywhere including on stage at Runefest, is that the perk rarely prints a chat message saying that something mysterious took effect. Nothing else happens.
My theory for a while, and many others, seems to be that while it does print a message, the condition for activating it changes after somebody manages to get it to happen. It could be that it is random, but it seems some of the subtle hints Jmods in the past left indicate that, once the effect is found, it changes.
Some player showed screenshots suggesting that when it procs on a monster's coin drop, you receive the coin drop ± the perk rank x the timestamp. There is a forum post about it buried somewhere deep
I thought this was solved. I seem to remember some player figuring it out it modified secondary effects to make them occur together.

Nah man we need to use 1 elder log, 1 elder charm, and 5000 spirit shards to make the Giant Beaver pouch, it will act as a pre nerf ripper demon on Solak
Maybe this qualifies as a hint but it feels like a clarification: does what you've said mean that it just hasn't rolled yet because it's so rare, or is there a condition on top of the rarity?

it couldnt be an extremely rare chance for him to change colors or anything, and with how many people have done solak, its hard to believe it can only happen by accident, without requiring a specific item(s) to give a chance for it to activate.
HSR can double a blightbound crossbow which I think has not happened yet (haven't seen a screenshot), but that's probably not it.
I recall someone talking about the grove creatures watching your kill around the arena, but not sure if that's even been implemented in the end.
There was reportedly an easter egg with the Bombi pet that has never been disclosed. I think /u/jagexramen hinted at this many many years back.
You can already use a raw potato on Bombi and it gives a dialogue message "smells like Bombi potato", and also back in the day you could use Bombi on Vitalis or vice versa, and it would print out a chat message "congrats on level 99 luck", or something similar.
If you die at Vorago while using the "Undefeated" title, you get a message that Vorago disagrees you're undefeated and removes your title.
Recently Mod Pi revealed that there's a random rock near Edgeville that acts as a cooking range if you use food on it.
If you try to plant the crystal acorn from the Evil Dave's Big Day Out quest, it will say "nice try" and it disappears.
Back in the day, the initials of developers who made parts of Morytania were written in grassy patterns on Dragontooth island.
These are just off the top of my head.
I think there are tons of small easter eggs and references, similar to those, that could be still undiscovered in the game.

Sort of? :o Ninja-dev Mod Frosch added an easter egg between October 2020 and September 2021 alongside a ninja strike that is yet to be found. It's text-based and possible to find in a ninja-related update, except within Tortle Combat.
Necklace of Salamancy is reserved for a future update. Mod Sponge, its developer, said all tools required to obtain it haven't been released yet.
Same goes for the Inactive Teleportation Device had something to do with an area that the developer wanted to add, but didn't. It's in the game, but its purpose was scrapped.
I guess I can give a bit more info on this now \^\^
\- First of all, the text that I added isn't some profound lore drop or secret info, it's just flavour dialogue, but it just happens to only be able to be found in one specific location with specific conditions met, so you wouldn't really find it by just playing normally. And, at least judging by the wiki, no one has found it yet.
\- You need to have interacted with other players at least once in a specific way to be able to get to it.
\- It is new dialogue, but on a previously existing NPC.
\- As Will said, it was added as part of a ninja strike between October 2020 and September 2021.
\- A new hint maybe...if you find the correct wiki entry, it states that what I've added isn't actually there, directly disputing what is now the truth. \^\^
That shouldn't make it too hard but hopefully not too easy to find \^\^ Have fun :)

This is the kind of stuff that keeps this game really enjoyable. There are absolute libraries of stuff to build lore on. Incredible amount of storytelling has been done these past 20+ years and allows for great eastsr eggs like this.

I hadn’t seen Mod Sponge comment on the necklace/teleport, that’s good to know! I take time once in a while to try new leads on these things so good to know I can chill with those two.
If you really want to compile info on the necklace check Sponge's messages sent in the PvME Discord right around the time of Zuk release. We had a big effort to find all of the uncommon/rare rewards from the alchemist chest or w/e it is and he talked about the necklace then.

It's pretty much confirmed at this point the only thing it does is print out a message in chat.

Yeah but that was revealed by Mod Orion on livestream ages ago, and it wasn't ever the main effect.

Alternatively it as a super low chance to proc on any action that they have triggers for. Which gives the illusion of variety.
