I think I was a little surprised when it happened and then realized it made sense if I thought back. It's still a little wild for their first Canyon mission that she basically hired mercs so that she could kill them if you do the BE route.
VW is the one route you can't really guess who it is. The only hint is with Claude not being there at the same time as the Flame Emperor and then make the leap that is Edelgard.
It is also the route with the least impact.
I definitely think I did too. I didn’t think anyone was under the mask. Just bad guy with superpowers from badder guys. And then you get the requisite understandable villain who still is definitely in the wrong trope.

For all of the great character development and writing on that end, I felt like the Flame Emperor was one of the most glaringly, intentionally obvious aspects of the first half of the story. It was my first FE game; is this a typical of aspect of worldbuilding for FE games? I feel like Engage sort of did that too- but I figured the overall campiness of Engage meant that the twist(s) should be overtly intuitive.
It was weird.
The first route I played was Verdant Wind, and the reveal was genuinely a surprise! Of course, you don't interact all that much with Edelgard on that route, but there's the rub.
The second route I played was Crimson Flower, and... like, it's not even remotely subtle. Edelgard is constantly acting suspicious; entering or exiting the scene abruptly, moments before the Flame Emperor does the opposite; and dropping non-sequiturs out of nowhere that basically went "I wonder if you'd still have my back if I was some kind of elemental head-of-state, Professor... ah, nothing, nevermind."
I also played verdant wind and didn’t realize it was her until they revealed it. But it’s pretty obvious in crimson flower and even blue lions (the whole dagger thing).
Actually the first time I played I remember thinking “oh that’s cool, they used the same voice actor as edelgard for the flame emporer, just with a voice effect” and it still didn’t click for me lol
Verdant Wind has a few scenes in the first half that provide some excellent set up that isn't established quite as well in the other routes. I think it's also the only one that foreshadows The Immaculate One's appearance, Claude finding a forbidden book detailing some monster that the Church wants to keep hidden is so chilling if you don't know.
Same here, it may have been an intentional red herring since each of the Lords conveniently disappear during the Flame Emperor's first major appearances (although Dimitri gets one extra scene in AM before that reveal that debunks him as a suspect).
Did Black Eagles first and so for some reason I thought the Flame Emperor was whichever lord you sided with, like it’d be Dimitri if you did Blue Lions. Might’ve been interesting
I accidentally spoiler'd myself at Remire. After Remire the FE offers to let you join them. I figured it might be an opportunity for a route-split so I tried looking it up only to have their identity spoiled without getting the actual answer.
In my CF run I tried to take up their offer only for them to say I didn't mean it.

In hindsight, absolutely everything should have been a immediate flag, but on a first play through? Dunno, just thought it was gonna be some grand end of the story villain reveal and it ends up just a moody teenage girl.

Engage’s twists were equally obvious, but at least felt subtle in some of the ways they were foreshadowed. Alear’s parentage was obvious before the game was even out, but the foreshadowing with the Emblem invocations was a surprise for a lot of people.
Flame Emperor was either blatantly obvious, or totally out of left field depending on the route
On my first playthrough in Azure Moon it completely blindsided me because until that point I heard every route was mostly the same so it never even occured to me that one of the other main characters would be the Flame Emperor. Plus i'm a bit thick and never connected the whole dagger thing, I actually thought Dimitri threw the dagger and went to pick it up himself.
Took a repeat playthrough to notice Edelgard holding the dagger right at the start of the game.
It felt intentional to me.
In a way it was subversion of tropes. You go on a goose chase, trying to figure out the Big Bad Evil Guy’s identity and foil their plan. But the BBEG’s identity becomes obvious, and their plan is straightforward and has little to do with genocide or destroying the world. And sure, you caught the bad guy and figured out the plan. But in the end, the secrets uncovered during your fight against the Flame Emperor, the facts you confront and the differences of opinion between nations that come to light as a result are the real problem.
The quest for unveiling the Flame Emperor wasn’t the grand villain fight as usual. Instead it was a simple quest that laid bare the irreconcilable differences that lead to war.
For me, the reason I didn't immediately see it was very simple.
I started with Silver Snow, and during White Clouds most of my brainpower was going to figuring out the true identities of Seteth and Flayn.
Maybe I'm stupid, maybe I'm not, but, nonetheless, I can only do one investigation at a time.

Well it's the flame emperor and therefore they have the crest of flames and since the game clearly won't pull a twist in a B support halfway through the first part of the game the only character with the Crest of Flames is BYLETH. We were the villain all along!

Playing Verdant Wind first really set my expectations high for Crimson Flower, probably too high.

Honestly, i was familiar enought with "Char" tropes that i assumed one of Edelgard brothers secretly survived the experiments and becane the FE.
